微创复位内固定治疗老年性Sanders Ⅱ、Ⅲ型跟骨骨折效果分析

摘　　要：目的:分析微创复位内固定在老年性Sanders Ⅱ、Ⅲ型跟骨骨折治疗中的有效性和优越性。方法:选取2018年7月-2019年6月于本院诊治的86例老年性Sanders Ⅱ、Ⅲ型跟骨骨折患者,应用双色球法分为对照组与观察组,各43例。对照组行切开复位内固定术,观察组行微创复位内固定术。随访1年,比较两组临床疗效、手术相关指标、术后跟骨解剖参数与术后并发症发生情况。结果:两组治疗有效率比较,差异无统计学意义(P>0.05)。观察组手术时间和住院时间均短于对照组,且术中出血量少于对照组(P<0.05)。术后1年,两组Bohler角和Gissane角均高于术前(P<0.05)。术后1年,两组Bohler角和Gissane角比较,差异均无统计学意义(P>0.05)。观察组术后并发症发生率为4.65%,明显低于对照组的18.60%(P<0.05)。结论:微创复位内固定术可缩短老年性Sanders Ⅱ、Ⅲ型跟骨骨折患者的手术时间与住院时间,减少术中出血量与术后并发症,临床应用价值高。Objective:To analyze the efficacy and superiority of minimally invasive reduction and internal fixation in the treatment of senile Sanders Ⅱ and Ⅲ calcaneal fractures.Method:A total of 86 patients with senile Sanders Ⅱ and Ⅲ calcaneal fractures in our hospital from July 2018 to June 2019 were selected.They were divided into control group and observation group,with 43 cases in each group.The control group was treated with open reduction and internal fixation,and the observation group was treated with minimally invasive reduction and internal fixation.Follow up for 1 year,the clinical efficacy,operation related indexes,postoperative calcaneal anatomic parameters and postoperative complications were compared between the two groups.Result:There was no significant difference in the effective rate between the two groups (P>0.05).The operative time and hospital stay of the observation group were lower than those of the control group,and the intraoperative blood loss was lower than that of the control group (P<0.05).1 year after surgery,Bohler angle and Gissane angle in the two groups were higher than those before surgery (P<0.05).1 year after operation,there were no statistically significant differences between the two groups in Bohler angle and Gissane angle (P>0.05).The incidence of postoperative complications in the observation group was 4.65%,it was significantly lower than 18.60% in the control group (P<0.05).Conclusion:Minimally invasive reduction and internal fixation can shorten the operation time and hospital stay of patients with senile Sanders Ⅱ and Ⅲ calcaneal fractures,reduce intraoperative blood loss and postoperative complications,it is has high clinical value.
