曼月乐联合GnRH-α治疗子宫腺肌症患者的临床疗效及对其卵巢功能、子宫体积的改善作用

摘　　要：目的探讨左炔诺孕酮宫内节育系统(曼月乐)联合促性腺激素释放激素激动剂(GnRH-α)治疗子宫腺肌症患者的临床疗效及对其卵巢功能、子宫体积的改善作用。方法选择我院2017年2月至2019年2月收治的96例子宫腺肌症患者作为研究对象,随机分为对照组与观察组,各48例。对照组行曼月乐治疗,观察组在对照组基础上增加GnRH-α治疗。比较两组患者月经量、痛经程度、子宫体积及子宫内膜厚度,评价临床疗效,观察不良反应发生情况。结果治疗后,观察组患者月经量、痛经程度、子宫体积及子宫内膜厚度、治疗总有效率、曼月乐环脱落发生率、阴道不规则出血发生率均优于对照组,两组数据比较,P<0.05。结论在子宫腺肌症患者治疗中采用曼月乐联合GnRH-α方案可促使患者恢复月经量,减轻痛经程度,并改善子宫体积及子宫内膜厚度,提高疗效,且可减少不良反应的发生,安全性高。Objective To investigate the clinical efficacy of levonorgestrel intrauterine contraceptive system(mirena)combined with gonadotropin releasing hormone agonist(GnRH-α)in the treatment of patients with adenomyosis and its effect on improving ovarian function and uterine volume.Methods Ninetysix cases of adenomyosis patients in our hospital from February 2017 to February 2019 were selected as the research objects and randomly divided into control group and observation group,48 cases in each group.The control group was treated with mirena,and the observation group was treated with GnRH-αon the basis of the control group.The menstrual volume,dysmenorrhea degree,uterine volume and endometrial thickness were compared between the two groups,to evaluate the clinical efficacy and observe the occurrence of adverse reactions.Results After treatment,the observation group patients'menstrual volume,dysmenorrhea,uterine volume and endometrial thickness,the total effective rate of treatment,the incidence of mirena ring shedding,and the incidence of irregular vaginal bleeding were better than those of the control group.Data comparison between the two groups,P<0.05.Conclusion In the treatment of patients with adenomyosis,mirena combined with GnRH-αcan promote the recovery of menstrual volume,reduce the degree of dysmenorrhea,improve the uterine volume and endometrial thickness,improve the curative effect,and reduce the occurrence of adverse reactions,with high safety.
