体外冲击波治疗神经根型颈椎病的经穴特异性反应研究

摘　　要：目的研究体外冲击波(ESW)技术在治疗神经根型颈椎病中列缺穴腧穴所发生的特异性反应。方法采集ESW对神经根型颈椎病患者手三阴经和手三阳经双侧各182个腧穴治疗前后的红外热成像数据及视觉模拟评分法(VAS)评定,通过统计分析列缺穴红外热成像的反应变化,分析列缺穴的经穴特异性与神经根型颈椎病之间的相关性。结果神经根型颈椎病患者VAS评分比较,冲击波治疗组治疗后较治疗前显著降低,具有统计学意义(P<0.01)。治疗前,健康对照组和冲击波治疗组同侧腧穴红外热成像温度进行独立样本t检验,具有统计学意义(P<0.05),治疗后,健康对照组和冲击波治疗组同侧腧穴红外热成像温度进行独立样本t检验,无统计学意义(P>0.05)。结论ESW对神经根型颈椎病患者进行治疗前后部分腧穴的体表温度存在变化,其中列缺穴的红外热成像反应最显著,提示列缺穴的经穴特异性反应与神经根型颈椎病之间存在相关性作用。Objective To study the specific response of extracorporeal shock wave(ESW)technology in the treatment of cervical spondylotic radiculopathy.Methods Collect the infrared thermal imaging data and Visual Analogue Scoring(VAS)evaluation of 182 points on each side of the three-yin meridian of the hand and the three-yang meridian of the hand in patients with cervical spondylotic radiculopathy before and after the treatment.Analyze the correlation between the specificity of the meridian points of lieque points and cervical spondylotic radiculopathy.Results Compared with the VAS scores of patients with cervical spondylotic radiculopathy,the shock wave treatment group was significantly lower than before treatment,which was statistically significant(P<0.01).Before treatment,the infrared thermal imaging temperature of the ipsilateral acupoints in the healthy control group and the shock wave treatment group was independently sampled by t-test,which was statistically significant(P<0.05).After treatment,the infrared thermal imaging temperature of the ipsilateral acupoints in the healthy control group and the shock wave treatment group was the independent sample t test was performed,and it was not statistically significant(P>0.05).Conclusion There are changes in the body surface temperature of some acupoints before and after the treatment of ESW in patients with cervical spondylotic radiculopathy.Among them,the infrared thermal imaging reaction of lieque point is the most significant,indicating that the specific response of the meridian points of lieque point is between the cervical spondylotic radiculopathy there is a correlation effect.
