摘　　要：目的:调查老年慢性病“知、信、行”现况,并以骨质疏松老年患者为例了解相关影响因素,探索个性化健康管理对骨质疏松“知、信、行”情况的影响,方法:选取海南省干部疗养院骨质疏松老年患者为研究对象,调查其对骨质疏松的“知、信、行”情况,通过多重线性回归分析相关的影响因素结果:本人及子女的文化程度较高、家庭中有医疗卫生技术人员、生活所在社区有骨质疏松知识宣教以及社区体检包括骨密度检测项目是老年患者对骨质疏松症“知、信、行”的有利因素。个性化健康管理后研究对象知信行得分及骨密度均明显改善,结论:老年患者及其子女文化水平、社区的骨质疏松知识宣教力度、社区体检包括骨密度检测是老年患者“知、信、行”的有利因素,老年人的健康管理政策应考虑这些因素。个性化的健康管理对防治老年患者骨质疏松具有较好效果。Objective To investigate the status of knowledge,attitude and behavior of the elderly patients with chronic diseases,explore its influencing factors,and explore the effect of personalized health management.Methods The elderly patients with osteoporosis were chosen from one hospital and surveyed.The influencing factors were analyzed with multiple linear regression.Results Multiple linear regression analysis showed that patients or their children with higher education background,having medical staffs in patients'family,osteoporosis knowledge education and bone mineral density test were favorable factors to obtain the knowledge,attitude and behavior of the elderly patients.Individualized health management can significantly improve the scores of knowledge,attitude and behavior and the bone mineral density.Conclusion The education level of the elderly patients and their children,osteoporosis knowledge propaganda and education in community,and bone mineral density test in community are important favorable factors.Personalized health management has good effect on the prevention and treatment of osteoporosis in elderly patients.
