摘 要:目的研究彩色多普勒超声应用于早期糖尿病肾病诊断中的价值。方法该次纳入2018年11月—2019年10月收治的60例早期糖尿病肾病患者、60名健康体检者展开研究,分别纳入观察组、对照组,两组均接受彩色多普勒超声检查。将两组的肾脏大小相关参数、肾脏各级动脉血流参数进行比对。结果观察组的肾脏长度、宽度、厚度、体积分别为(108.62±8.49)mm、(57.56±5.40)mm、(58.19±7.06)mm、(151.32±24.63)mm3,均大于对照组的(96.07±7.35)mm、(48.68±5.19)mm、(47.30±6.25)mm、(120.06±20.47)mm3,差异有统计学意义(t=8.657、9.184、8.946、7.561,P<0.05);观察组MRA、SRA、IRA的Vs(61.87±13.05)、(32.60±7.15)、(22.46±4.65)cm/s、Vd(18.40±2.43)、(13.29±2.37)、(12.75±3.88)cm/s均低于对照组的Vs(70.35±14.67)、(50.98±7.64)、(32.71±5.76)cm/s、Vd(22.73±4.24)、(20.46±3.42)、(18.96±4.12)cm/s,差异有统计学意义(t=3.345、13.606、10.725、6.863、13.348、8.500,P<0.05),观察组的RI(0.70±0.08)、(0.67±0.05)、(0.66±0.07)高于对照组(0.60±0.05)、(0.62±0.04)、(0.54±0.05),差异有统计学意义(t=8.211、6.049、10.805,P<0.05)。结论彩色多普勒超声能够通过对肾脏形态、肾脏血流参数的反映准确诊断早期糖尿病肾病,具有较高的临床诊断价值。Objective To study the value of color Doppler ultrasound in the diagnosis of early diabetic nephropathy.Methods In this study,60 patients with early diabetic nephropathy and 60 healthy subjects who were admitted from November 2018 to October 2019 were included in the study.They were included in the observation group and the control group.Both groups received color Doppler ultrasound.The parameters related to kidney size and arterial blood flow parameters at all levels of the kidneys were compared between the two groups.Results The length,width,thickness and volume of the kidneys in the observation group were(108.62±8.49)mm,(57.56±5.40)mm,(58.19±7.06)mm,and(151.32±24.63)mm^3 respectively,which were larger than those of the control group(96.07±7.35)mm,(48.68±5.19)mm,(47.30±6.25)mm,(120.06±20.47)mm^3,the difference was statistically significant(t=8.657,9.184,8.946,7.561,P<0.05);MRA of observation group,SRA,IRA Vs(61.87±13.05)cm/s,(32.60±7.15)cm/s,(22.46±4.65)cm/s,Vd(18.40±2.43)cm/s,(13.29±2.37)cm/s,(2.75±3.88)cm/s were all lower than the Vs of the control group(70.35±14.67)cm/s,(50.98±7.64)cm/s,(32.71±5.76)cm/s,Vd(22.73±4.24)cm/s,(20.46±3.42)cm/s,(18.96±4.12)cm/s,the difference was statistically significant(t=3.345,13.606,10.725,6.863,13.348,8.500,P<0.05),the RI of the observation group(0.70±0.08),(0.67±0.05),(0.66±0.07)was higher than In the control group(0.60±0.05),(0.62±0.04),(0.54±0.05),the difference was statistically significant(t=8.211,6.049,10.805,P<0.05).Conclusion Color Doppler ultrasound can accurately diagnose early diabetic nephropathy through the reflection of kidney morphology and renal blood flow parameters,and has high clinical diagnostic value.
