超声心动图诊断飞行员心脏室间隔膜部瘤1例并文献复习

摘　　要：目的探讨飞行员心脏室间隔膜部瘤的彩色多普勒超声心动图诊断及临床意义。方法回顾性分析1例飞行员心脏室间隔膜部瘤的超声图像特征并复习相关文献,总结心脏室间隔膜部瘤的分型、诊断、临床意义以及其航空医学鉴定。结果彩色多普勒超声显示室间隔膜部凸向右室,呈网兜样改变,大小约0.8 cm×1.0 cm,CDFI未见过隔血流信号。超声诊断:室间隔膜部瘤。特许飞行医学鉴定,飞行合格。结论彩色多普勒超声心动图检查能够清晰显示室间隔膜部病变情况,并可追踪观察膜部瘤形成形态、大小,超声诊断与冠脉CTA相符,具有较高的临床价值。室间隔膜部瘤飞行员无任何不适主诉和体征,各项心血管疾病危险因素控制良好,心功能良好,建议密切观察并定期复查、追踪随访。Objective To investigate the diagnosis and clinical significance of color Doppler echocardiography in pilots'cardiac septal tumors.Methods A retrospective analysis of the ultrasound image characteristics of a pilot's cardiac septal tumor and review of related literature,summarized the classification,diagnosis,clinical significance and aviation medical identification of cardiac ventricular septal tumor.Results Color Doppler ultrasound showed that the interventricular septal membrane protruded to the right ventricle,showing a net bag-like change,with a size of about 0.8 cm×1.0 cm.CDFI did not see the septal blood flow signal.Ultrasound diagnosis:interventricular septal tumor.Licensed flight medical appraisal,qualified flight.Conclusion Color Doppler echocardiography can clearly show the lesions of the interventricular septum,and can track the morphology and size of the membranous tumors.Ultrasound diagnosis is consistent with coronary CTA and has high clinical value.Pilots with ventricular septal tumors have no complaints or signs of discomfort,various cardiovascular disease risk factors are well controlled,and heart function is good.Close observation,regular review,and follow-up are recommended.
