温针灸循经筋阿是穴联合独活寄生汤治疗膝骨关节炎疗效观察

摘　　要：目的观察温针灸循经筋阿是穴联合独活寄生汤内服治疗膝骨性关节炎的疗效及对患者血清白介素-1β(IL-1β)、白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)的影响。方法将86例膝骨性关节炎患者随机分为研究组43例和对照组43例,对照组采用独活寄生汤内服治疗,研究组采用温针灸循经筋阿是穴联合独活寄生汤内服治疗的方法。4周为1个疗程,治疗1个疗程后观察视觉模拟量表(VAS)及骨性关节炎指数(WOMAC)评分变化及患者血清中IL-1β、IL-6与TNF-α水平变化,并评定疗效。结果治疗后,两组VAS及WOMAC评分均降低,且差异具有统计学意义(P<0.05);两组患者血清中IL-1β、IL-6与TNF-α较前明显降低,且研究组优于对照组,差异具有统计学意义(P<0.05);综合比较后,对照组总有效率为65.11%,研究组总有效率为90.69%,研究组与对照组疗效差别具有统计学意义。结论温针灸循经筋阿是穴联合独活寄生汤内服可以明显降低膝骨性关节炎患者VAS及WOMAC评分,缓解疼痛、僵硬等症状,下调血清中IL-1β、IL-6与TNF-α水平,并且改善患者生活质量。推测作用机制可能与减低IL-1β、IL-6与TNF-α水平,调节炎症机制有关。Objective To observe the therapeutic effect of warming acupuncture at Ashi point along meridians combined with Duhuo Jisheng Decoction on knee osteoarthritis and the effect on interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α).Methods 86 patients with knee osteoarthritis were randomly divided into study group(43 cases)and control group(43 cases).The control group was treated with Duhuo Jisheng Decoction,and the study group was treated with warming acupuncture at Ashi point along meridian combined with Duhuo Jisheng Decoction.After one course of treatment,the changes of visual analog scales(VAS),Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)and the levels of IL-1β,IL-6 and TNF-αin patients’serum were observed,and the therapeutic effect was evaluated.Results After treatment,the VAS and WOMAC scores of the two groups were reduced,and the difference was statistically significant(P<0.05);the serum IL-1β,IL-6 and TNF-αof the two groups were significantly lower than those of the former,and the difference was statistically significant(P<0.05);after comprehensive comparison,the total effective rate of the control group was 65.11%,the total effective rate of the study group was 90.69%,and the difference between the study group and the control group was statistically significant(P<0.05).It has statistical significance.Conclusion The combination of warming acupuncture at Ashi point along meridian combine with Duhuo Jisheng Decoction can significantly reduce the VAS and WOMAC scores,relieve pain,stiffness and other symptoms,reduce the levels of IL-1β,IL-6 and TNF-αin serum,and improve the quality of life of patients.It is speculated that the mechanism of action may be related to the reduction of IL-1β,IL-6 and TNF-αlevels and the regulation of inflammatory mechanism.
