显微镜下根尖屏障术治疗根尖孔敞开恒牙的效果探讨

摘　　要：目的对显微镜下根尖屏障术治疗根尖孔敞开恒牙的效果进行评析。方法方便选取2016年9月—2019年9月该院口腔科收治的63例(均是单颗患牙)无牙髓活力根尖孔敞开恒牙患者纳入观察中,均行显微镜下根尖屏障术治疗,掌握填充效果,随访3个月评估临床效果,并评测治疗前后患牙咬合力和软组织红色美学指数评分(PES)。结果该组63例患者均一次性完成手术,术后复查恰填58例,4例超填,1例欠填;术后随访3个月,有效率为100.00%,未出现明显不适。术后随访评估,治疗后1个月、3个月PES评分分别为(10.18±1.26)分、(11.05±1.12)分,高于治疗前(8.26±1.10)分,差异有统计学意义(Z=5.332,P<0.05);治疗后1个月、3个月的咬合力分别为(134.15±18.40)lps、(135.06±19.17)lps,均显著高于治疗前的(95.71±21.30)lps(Z=8.054,P<0.05)。结论采用显微镜下根尖术治疗根尖敞开恒牙有着良好效果,且可实现较理想的软组织美观度和咬合力,值得临床应用。Objective To evaluate the effect of apical barrier treatment under the microscope for open apical foramen permanent teeth.Methods Convenient selection 63 cases of permanent teeth with open apical foramen without pulp vitality admitted to the Department of Stomatology in the hospital from September 2016 to September 2019 were included in the observation,all under the microscope apical barrier operate treatment,grasp the filling effect,follow up for 3 months to evaluate the clinical effect,and evaluate the bite force and soft tissue red aesthetic index score(PES)of the affected tooth before and after treatment.Results All 63 patients in this group completed the operation at one time,58 cases were rechecked after operation,4 cases were overfilled,and 1 case was underfilled.The postoperative follow-up was 3 months,the effective rate was 100.00%,and there was no obvious discomfort.Postoperative follow-up evaluation showed that PES scores were(10.18±1.26)points and(11.05±1.12)points 1 month and 3 months after treatment,which were higher than those before treatment(8.26±1.10)points,the difference was statistically significant(Z=5.332,P<0.05);The bite force at 1 month and 3 months after treatment were(134.15±18.40)lps and(135.06±19.17)lps,which were significantly higher than those before treatment(95.71±21.30)lps(Z=8.054,P<0.05).Conclusion The use of microscopic apical surgery to treat open apical permanent teeth has a good effect,and can achieve ideal soft tissue aesthetics and occlusal force,which is worthy of clinical application.
