摘 要:目的:探讨多模态监测治疗重型颅脑损伤效果分析。方法:将2017年1月~2019年1月在某院神经外科ICU治疗的70例重型颅脑损伤患者随机分为观察组和对照组,对照组单行常规生命体征监测,观察组行多模态监测,比较两组患者治疗前后的脑氧分压值及颅内压变化、治疗1周内甘露醇用量、治疗前后的脑血流变化、伤后6个月GOS疗效。结果:观察组治疗后72h的脑氧分压值明显高于对照组,而颅内压值明显低于对照组(P<0.05);观察组治疗1周后的甘露醇用量明显少于对照组(P<0.05);观察组治疗1周后的MCA、PCA、ACA、VA、BA均明显高于对照组(P<0.05);观察组伤后6个月GOS良好率明显高于对照组(P<0.05)。结论:多模态监测治疗重型颅脑损伤效果显著,有助于指导临床治疗和用药,促进病情的缓解,改善预后,具有积极的意义。Objective:To explore the effect of multimodal monitoring in the treatment of severe craniocerebral injury.Methods:70 cases of severe craniocerebral injury patients treated in a hospital from January 2017 to January 2019 were selected and randomly divided into two groups.The control group was given routine vital signs monitoring,and the observation group was given multi-modal monitoring,then the changes of cerebral oxygen partial pressure and intracranial pressure before and after treatment,dosage of mannitol within 1 week of treatment,changes of cerebral blood flow before and after treatment,GOS curative effect at 6 months after injury were compared between the two groups.Results:The brain oxygen partial pressure value of 72 h after treatment in the observation group was significantly higher than that in the control group,while the intracranial pressure value was significantly lower than that in the control group(P<0.05);The mannitol dosage within 1 week in the observation group was significantly lower than that in the control group(P<0.05).The MCA,PCA,ACA,VA and BA in the observation group were significantly higher than those in the control group after 1 week of treatment(P<0.05).The GOS good rate at 6 months after injury in the observation group was significantly higher than that in the control group(P<0.05).Conclusion:Multimodal monitoring has a significant effect in the treatment of severe craniocerebral injury,which is helpful for guiding clinical treatment and medication,promoting the remission of the disease and improving the prognosis,with positive significance.
