彩色多普勒超声在血液透析患者自体动静脉内瘘中的应用研究

摘　　要：目的:探讨采用彩色多普勒超声在血液透析患者自体动静脉内瘘的应用效果。方法:选择2016年8月~2018年8月期间于某院长期接受血液透析患者100例,采用彩色多普勒超声检查,对于患者桡动脉自体动静脉内瘘情况进行检查分析,并对患者并发症情况进行统计。结果:100例血液透析患者中,自体动静脉内瘘通畅患者66例,出现并发症患者34例,其中主要包括形成血栓患者19例,内瘘狭窄患者10例,盗血综合征患者5例,患者并发症发生率为24%;同时自体动静脉内瘘通畅患者桡动脉直径、峰值流速均显著高于并发症患者(P<0.05),且患者阻力指数比较差异不明显(P>0.05)。结论:采用彩色多普勒超声检查能够有效判断患者并发症发生情况以及患者自体动静内瘘通畅情况,对于及时发现患者内瘘并发症,及时介入相应治疗措施,提升患者生存质量具有重要意义。Objective:To explore the application effect of color Doppler ultrasound in autologous arteriovenous fistula in hemodialysis patients.Methods:100 patients with long-term hemodialysis in a hospital from August 2016 to August 2018 were selected and examined by color Doppler ultrasonography.The internal impotence of radial artery autogenous arteriovenous was examined and analyzed,and the complications were counted.Results:Among the 100 hemodialysis patients,66 were autogenous arteriovenous unobstructed,and 34 had complications,including 19 cases of thrombosis,10 cases of internal impotence stenosis,5 cases of steal syndrome,the incidence of complications was 24%.Meanwhile,the diameter and peak flow velocity of radial artery in autogenous arteriovenous unobstructed patients were significantly higher than those in patients with complications(P<0.05),and there was no significant difference in resistance index(P>0.05).Conclusion:Color Doppler ultrasonography can effectively judge the occurrence of complications and the patency of autologous arteriovenous fistula.It is of great significance for timely detection of complications of impotence,timely intervention of corresponding treatment measures and improvement of patients'quality of life.
