银质针联合椎间孔注射治疗腰椎间盘突出症的临床研究

摘　　要：目的:研究银质针联合椎间孔注射治疗腰椎间盘突出症的临床疗效。方法:选择我科80例腰椎间盘突出症病人,随机分为对照组(40例)和观察组(40例),对照组采用CT引导下经椎间孔注射治疗,观察组采用银质针联合CT引导下经椎间孔注射治疗,采用视觉模拟评分法(visual analogue scale,VAS)评估治疗前、治疗后1月、3月的疼痛变化;采用MacNab疗效标准评估治疗后1月、3月的疗效。结果:两组治疗前VAS评分比较,差异无统计学意义(P>0.05),观察组治疗后1月、3月VAS评分均显著低于对照组,差异均有统计学意义(P<0.05)。两组治疗后1月、3月VAS评分均显著低于治疗前,差异均有统计学意义(P<0.05)。观察组治疗后1月、3月的优良率分别为80.0%、92.5%,均显著高于对照组(62.5%、77.5%)。结论:CT引导下经椎间孔注射治疗联合银质针综合治疗较单纯椎间孔注射治疗,疼痛缓解更明显、症状改善更显著,是安全可靠、操作简单的非手术治疗方法,应用价值高。Objective:To observe the clinical efficiency of the treatment of prolapse of lumbar intervertebral disc with silver acupuncture therapy combined with CT-guided transforaminal intervertebral foramen injection.Methods:Eighty patients diagnosed with prolapse of lumbar intervertebral disc were randomly divided into control group and observation group(n=40 each).Patients in the control group received CT-guided transforaminal intervertebral foramen injection,and patients in the observation group received silver acupuncture therapy combined with CT-guided transforaminal intervertebral foramen injection.The visual analogue scale(VAS)was used to evaluate the pain changes before treatment,1 month and 3 months after treatment.The efficacy of 1 month and 3 months after treatment was evaluated with reference to the MacNab efficacy standard.Results:There was no significant difference in VAS score between the groups before treatment(P>0.05),but VAS scores in the observation group at 1 and 3 months after treatment were significantly lower than those in control group(P<0.05).VAS scores in the two groups at 1 and 3 months after treatment were significantly lower than those before treatment(P<0.05).The excellent rate of 1 month and 3 months after treatment in the control group was 80%and 92.5%,respectively,which were significantly higher than those of the observation group(62.5%,77.5%).Conclusion:Treatment of silver acupuncture therapy combined with CT-guided transforaminal intervertebral foramen injection is more effective than simple minimally invasive treatment.It is a comprehensive treatment with quick pain relief,relatively safe operation and significant curative effect,and has high application value.
