比较外侧小切口髋关节置换术与后路小切口髋关节置换术治疗老年创伤性股骨颈骨折临床疗效

摘　　要：目的分析比较外侧小切口髋关节置换术与后路小切口髋关节置换术治疗老年创伤性股骨颈骨折临床疗效。方法在该院患者中方便选取2017年10月—2019年10月共60例老年创伤性股骨颈骨折患者作为该次研究对象,以电脑随机的方法将他们对半分为对照组和实验组。对照组采用外侧小切口髋关节置换术治疗,实验组采用后路小切口髋关节置换术治疗。比较两组患者的疼痛视觉模拟评分法(VAS)评分和治疗效果,比较两组患者的髋关节功能恢复时间。结果实验组患者的VAS评分(3.5±1.2)分低于对照组(4.5±1.8)分,差异有统计学意义(t=2.530,P=0.010)。实验组患者的治疗有效率(93.33%)高于对照组(73.33%),差异有统计学意义(χ^2=4.320,P=0.030)。实验组患者的髋关节功能恢复时间(12.1±2.1)d短于对照组(22.2±2.5)d,差异有统计学意义(t=16.940,P<0.001)。结论后路小切口髋关节置换术应用于老年创伤性股骨颈骨折患者的治疗可以取得不错的治疗效果,具有推广价值。Objective To analyze and compare the clinical efficacy of lateral small incision hip arthroplasty and posterior small incision hip arthroplasty in the treatment of elderly traumatic femoral neck fractures. Methods A total of 60 elderly patients with traumatic femoral neck fractures from October 2017 to October 2019 were convenienty selected from the patients in the hospital as the subjects of this study. They were divided into the control group and the experimental group in half by computer randomization. The control group was treated with lateral small incision hip replacement, and the experimental group was treated with posterior small incision hip replacement. The pain visual analogue scale(VAS) scores and treatment effects of the two groups of patients were compared, and the hip function recovery time of the two groups of patients was compared. Results The VAS score(3.5±1.2)points of the experimental group was lower than that of the control group(4.5±1.8)points,the difference was statistically significant(t=2.530, P=0.010). The effective rate of treatment in the experimental group(93.33%) was higher than that of the control group(73.33%),the difference was statistically significant(χ^2=4.320, P=0.030). The recovery time of hip joint function of the experimental group(12.1±2.1)d was lower than that of the control group(22.2±2.5)d,the difference was statistically significant(t=16.940, P<0.001). Conclusion The posterior small incision hip arthroplasty applied to the treatment of elderly patients with traumatic femoral neck fractures can achieve good therapeutic effects, and has promotion value.
