比较双极人工股骨头置换术与全髋关节置换术治疗老年股骨颈骨折疗效

摘　　要：目的对比双极人工股骨头置换术和全髋关节置换术就老年股骨颈骨折患者中的治疗效果。方法便利收集2015年1月—2019年6月期间该院接收的股骨颈骨折患者73例的治疗信息,其中35例行双极人工股骨头置换术为对照组,38例行全髋关节置换术为观察组,对比分析两组手术指标、治疗有效率及术后1年和3年的Harris评分。结果观察组各项手术指标[(109.6±22.1)min、(351.3±39.1)mL、(284.3±32.4)mL]均显著高于对照组[(84.7±14.1)min、(133.5±17.3)mL、(88.4±16.2)mL](t=6.939、30.320、32.240);且该组临床疗效(97.37%)与术后3年Harris评分(79.1±4.1)分则明显优于对照组[(77.14%、(72.0±3.8)分](χ^2=3.989,t=7.655),差异有统计学意义(P<0.05)。结论 BFHR双极人工股骨头置换术具有手术时间短、创伤小等优点,但其远期治疗效果相对较差;THR全髋关节置换术虽然在手术时间上相对较长,手术过程中对患者造成的创伤大,但其远期治疗效果较好。Objective To compare the therapeutic effects of bipolar artificial femoral head replacement and total hip replacement in elderly patients with femoral neck fracture. Methods The treatment information of 73 patients with femoral neck fractures received in the hospital from January 2015 to June 2019 was convenienty collected. Among them, 35 patients underwent bipolar artificial femoral head replacement as the control group, and 38 patients underwent total hip replacement as the observation group, the surgical indicators, the effective rate of treatment and the Harris scores at 1 and 3 years after the operation were compared and analyzed between the two groups. Results The operation indexes of the observation group[(109.6±22.1)min,(351.3±39.1)mL,(284.3±32.4)mL] were significantly higher than those of the control group[(84.7±14.1)min,(133.5±17.3)min,(88.4±16.2)]mL(t=6.939,30.320,32.240,P<0.05);In addition, the clinical efficacy(97.37%) and Harris score(79.1 ±4.1) points of this group at 3 years after surgery were significantly better than those of the control group [77.14%,(72.0 ±3.8)points ](χ^2=3.689, t =7.655). The difference was statistically significant(P <0.05). Conclusion BFHR bipolar artificial femoral head replacement has the advantages of short operation time and less trauma, but its long-term treatment effect is relatively poor. Although THR total hip replacement has relatively long operation time, it will cause large trauma to patients during the operation, but its long-term treatment effect is good, and it is worth promoting.
