摘 要:目的对比分析不同时段胰岛素泵基础量设置对2型糖尿病患者血糖控制的临床效果。方法选择2018年12月—2019年10月该院收治的120例2型糖尿病患者为研究对象,以随机数表法将其分为3组,每组各40例,A组将胰岛素的基础量分3个时段泵入;B组分5个时段泵入,C组分24个时段泵入,比较治疗前后3组患者血糖水平变化情况、血糖达标时间以及低血糖反应发生情况。结果治疗后,3组患者的空腹血糖、早饭后2 h血糖、午饭后2 h血糖、晚饭后2 h血糖及凌晨3点血糖水平均较治疗前显著减少(P<0.05),但组间差异无统计学意义(P>0.05)。C组、B组患者血糖达标时间、胰岛素日用量、低血糖反应发生率均明显少于A组(P<0.05),B、C组的低血糖反应发生率对比差异无统计学意义(P>0.05)。结论3种不同胰岛素泵基础量分段法对2型糖尿病患者的血糖水平控制效果相当,但24段法胰岛素疗法能够显著缩短病程时间,降低低血糖反应发生率,但其实际操作繁杂,而5段法的操作相对较为简单,且其低血糖反应发生率与24段法无明显差异,故5段法可作为胰岛素泵治疗2型糖尿病的首选设置方法。Objective To compare and analyze the clinical effects of insulin pump basal volume setting in different time periods on blood glucose control in patients with type 2 diabetes.Methods A total of 120 patients with type 2 diabetes who were admitted to the hospital from December 2018 to October 2019 were selected as the research objects.They were divided into 3 groups by the random number table method,each with 40 cases,and the basic amount of insulin in group A pumped in 3 periods;component B is pumped in 5 periods,and component C is pumped in 24 periods.Compare the changes in blood glucose levels,time to reach the target blood glucose level and occurrence of hypoglycemia in the three groups before and after treatment.Results After treatment,the fasting blood glucose,2 h blood glucose after breakfast,2 h blood glucose after lunch,2 h blood glucose after dinner,and 3 am blood glucose levels of the three groups were significantly lower than before treatment(P<0.05),but there was no statistically significant difference between the groups(P>0.05).The blood glucose reaching standard time,daily insulin dosage,and the incidence of hypoglycemia in group C and B were significantly less than those in group A(P<0.05).There was no statistically significant difference in the incidence of hypoglycemia in groups B and C(P>0.05).Conclusion The three different insulin pump basic dose segmentation methods have equivalent effects on the control of blood glucose levels in patients with type 2 diabetes,but the 24-segment insulin therapy can significantly shorten the course of the disease and reduce the incidence of hypoglycemia,but the actual operation is complicated,and 5 segment method is relatively simple,and the incidence of hypoglycemia is not significantly different from that of the 24-segment method,so the 5-segment method can be used as the preferred setting method for insulin pump treatment of type 2 diabetes.
