DDA入路切开复位空心钉结合内侧支撑钢板内固定与闭合复位空心钉内固定治疗股骨颈骨折的比较研究

摘　　要：目的比较DDA入路切开复位3根空心钉结合内侧支撑钢板内固定与闭合复位3根空心钉内固定治疗股骨颈骨折的临床疗效。方法将自2014年1月—2018年6月中青年移位型股骨颈骨折分为两组,分别使用DDA入路切开复位3根空心钉结合内侧支撑钢板内固定(A组)与闭合复位3根空心钉内固定(B组)。比较两组手术时间,术中出血量、骨折复位质量、术后并发症。结果 B组手术时间和术中出血量(67.2±12.9)min,(85.7±31.7)mL优于A组(84.7±13.1)min,(166.7±48.5)mL,差异有统计学意义(t=4.298、6.448,P=0.001、0.001)。A组并发症11.11%;B组26.09%,差异有统计学意义(χ^2=5.110,P=0.002),A组骨折复位质量优于B组,差异有统计学意义(χ^2=9.417,P=0.003)。结论 DDA入路切开复位3根空心钉结合内侧支撑钢板内固定比较闭合复位3根空心钉内固定治疗股骨颈骨折复位质量更高,骨折不愈合、股骨头无菌坏死及退钉的发生率较低。Objective To compare the clinical effects of open reduction with three hollow nails combined with medial support plate internal fixation and closed reduction with three hollow nails for the treatment of femoral neck fractures.Methods From January 2014 to June 2018, young and middle-aged displaced femoral neck fractures were divided into two groups, and three hollow nails combined with medial support plate internal fixation(group A) and three closed reductions were used for open reduction and DDA approach;and hollow nail internal fixation(group B). The operation time, intraoperative blood loss, fracture reduction quality, and postoperative complications were compared between the two groups. Results Operation time and intraoperative blood loss in group B were(67.2±12.9) min,(85.7±31.7) mL was better than group A(84.7±13.1) min,(166.7±48.5)mL,the difference was statistically significant(t=4.298, 6.448, P=0.001, 0.001). Complications in group A were 11.11%;group B was 26.09%,the difference was statistically significant(χ^2=5.110,P =0.002). The fracture reduction quality of group A was better than that of group B,the difference was statistically significant(χ^2=9.417, P =0.003). Conclusion DDA Compared with closed reduction, three hollow nails combined with medial support plate internal fixation for the treatment of femoral neck fractures, open reduction and open reduction with three hollow nails are of higher quality. The incidence of nonunion fractures, aseptic necrosis of the femoral head, and nail withdrawal are lower.
