摘　　要：肛瘘作为肛肠科的常见病及多发病,存在临床诊疗方案繁多、认知不统一等问题,缺乏业界公认的诊疗规范,多数研究仅限于理论探讨,临床指导意义较弱,导致肛瘘术后复发率较高。北京肛肠学会通过问卷调查的方式征询了国内肛肠领域30余位专家的意见,结合循证医学证据进行反复推敲,形成了《肛瘘的诊治专家共识(2020版)》。本共识提出了肛瘘的治疗原则,指出了传统挂线、肛瘘切开术、Lift术式、瘘管填塞术及近年兴起的视频辅助下肛瘘切除术(VAAFT)等多种术式的优缺点,以指导肛肠专业医生的临床实践,规范诊疗行为,提高肛瘘治愈率、减少复发率。Anal fistula,a common and frequently-occurring disease in the anorectal department,has numerous clinical diagnosis and treatment schemes and inconsistent modes of cognition.Most of the studies are limited to theoretical discussion,which leads to a high recurrence rate of anal fistula due to the poor instruction significance. Therefore,Beijing Anorectal Society has consulted more than 30 domestic experts in the anorectal field through questionnaire survey. Through repeated discussions on the basis of evidence-based medicine,the Consensus of Experts on Diagnosis and Treatment of Anal Fistula( 2020 Edition) was formed. The consensus puts forward the principle of anal fistula treatment,and points out advantages and disadvantages of various surgical procedures such as traditional thread-ligating therapy,anal fistula incision operation,Lift operation,fistula filling surgery,and video-assisted anal fistula resection( VAAFT) emerging in recent years so as to provide guidance for the clinical practice of professional anorectal doctors,standardize the diagnosis and treatment,improve the cure rate of anal fistula and reduce the recurrence rate.
