摘　　要：目的:分析Peguero-Lo Presti指数对高血压性左心室肥厚(LVH)的诊断价值。方法:选取原发性高血压患者309例,描记标准十二导联心电图(ECG),分别计算Sokolow、Cornell及Peguero-Lo Presti指数。同时行心脏核磁共振成像(CMR)、超声心动图(UCG)检查,以CMR作为诊断LVH的参考标准,LVH组226例(男178例,女48例),非LVH组83例(男45例,女38例),计算以上ECG指数及UCG用于诊断LVH的灵敏度和特异度,并计算约登指数(YI)。结果:常规ECG诊断LVH,在男性患者中,Peguero-Lo Presti指数诊断LVH灵敏度高于Sokolow指数及Cornell指数(P<0.001),其特异度略低于Cornell指数(P=0.012),且在3种指数中YI最高;而在女性患者中,Peguero-Lo Presti指数诊断LVH灵敏度高于Sokolow指数(P=0.010)与Cornell指数(P<0.001),特异度略低于Cornell指数(P=0.004),YI较Sokolow、Cornell指数略低。与UCG比较,在男、女性患者中,Peguero-Lo Presti指数诊断LVH的YI均稍偏低。结论:在高血压性LVH的诊断中,ECG Peguero-Lo Presti指数不仅灵敏度较高,并且在男性患者中具有较好的检测效果。Aim:To analyze the diagnostic value of Peguero-Lo Presti criteria in hypertensive left ventricular hypertrophy(LVH).Methods:A total of 309 essential hypertension patients were selected with electrocardiogram(ECG),ultrasonic cardiogram(UCG)and cardiac magnetic resonance(CMR).With CMR as the reference to diagnose LVH,226 LVH patients(178 males and 48 females)and 83 non-LVH patients(45 males and 38 females)were collected.Sokolow criteria,Cornell criteria,Peguero-Lo Presti criteria,and the sensitivity,specificity,Youden index(YI)of these criteria for diagnosing LVH were calculated.Results:Peguero-Lo Presti criteria had a higher sensitivity among these criteria(P<0.001)but a lower specificity comparted with Cornell criteria(P=0.012),with the highest YI in diagnosing LVH among males.For female patients,Peguero-Lo Presti criteria had a higher sensitivity compared with Sokolow criteria(P=0.010)and Cornell criteria(P<0.001),and a slightly lower specificity compared with Cornell index(P=0.004),and its YI was slightly lower in diagnosing LVH.Compared with UCG,the YI of Peguero-Lo Presti criteria was slightly lower in male and female patients.Conclusions:Peguero-Lo Presti criteria has higher sensitivity with better effects in diagnosing LVH for male patients.
