Effect of AQUACEL Ag Dressing Combined with Ji'anshuneng in the Treatment of Deep Second Degree Burn Wounds

摘　　要：目的:探讨爱康肤银联合济安舒能治疗深Ⅱ度烧伤创面的临床价值。方法:纳入2017年1月-2019年1月笔者医院收治的74例深Ⅱ度烧伤患者为研究对象,按照随机数表法分为观察组和对照组,各37例,其中观察组采用爱康肤银离子敷料联合济安舒能进行治疗,对照组采用传统磺胺嘧啶银霜进行治疗。比较两组创面愈合时间、创面渗出量、创面换药次数、住院时间、医疗总费用及美容效果,记录血清血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)含量、创面细菌阳性检出率及不同时点创面疼痛评分,观察治疗期间创面周围皮肤情况。结果:观察组创面愈合时间、创面渗出量、创面换药次数、住院时间、医疗总费用显著低于对照组,差异有统计学意义(P<0.05)。观察组治疗后美容效果优良率为94.59%显著高于对照组的78.38%,差异有统计学意义(P<0.05)。两组治疗后血清VEGF、bFGF含量较治疗前显著升高,观察组治疗后血清VEGF、bFGF含量显著高于对照组,差异有统计学意义(P<0.05)。两组治疗后14d创面细菌阳性检出率较治疗后7d显著降低,且观察组治疗后7d、14d创面细菌阳性检出率显著低于对照组,差异有统计学意义(P<0.05)。两组敷料移除中创面疼痛评分较敷料移除前显著降低,敷料移除后创面疼痛评分较敷料移除前、敷料移除中显著降低,且观察组敷料移除中、敷料移除后创面疼痛评分显著低于对照组,差异有统计学意义(P<0.05)。观察组治疗过程中未出现创面药物浸渍,对照组创面药物浸渍发生率为29.73%(11/37),差异有统计学意义(P<0.05)。结论:爱康肤银联合济安舒能治疗深Ⅱ度烧伤创面,能加快患者创面愈合,降低细菌感染风险,并减轻治疗过程中创面疼痛感,提高美容效果,具有较高临床应用价值。Objective To study the clinical value of AQUACEL Agdressing combined with Ji'anshuneng in the treatment of deep second degree burns.Methods 74 patients with deep second degree burn admitted to our hospital from January 2017 to January 2019 were included as subjects.According to the random number table method,they were divided into observation group and control group,37 cases in each group.The observation group was treated with AQUACEL Ag dressing combined with Ji'anshuneng,and the control group was treated with traditional sulfadiazine silver cream.The time of wound healing,the amount of wound exudation,the times of dressing change,the time of hospitalization,the total cost of medical treatment and the effect of Cosmetology were compared between the two groups.Results The time of wound healing,the amount of exudation,the times of dressing change,the time of hospitalization and the total cost of medical treatment in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The excellent and good rate of cosmetic effect in the observation group was 94.59%,which was significantly higher than 78.38%in the control group(P<0.05).After treatment,the serum levels of vascular endothelial growth factor and bFGF in the two groups were significantly higher than those before treatment(P<0.05).The serum levels of vascular endothelial growth factor and bFGF in the observation group were significantly higher than those in the control group(P<0.05).The positive rate of bacteria in wounds of the two groups on the 14th day after treatment was significantly lower than that on the 7th day after treatment(P<0.05).The positive rate of bacteria in wounds of observation group was significantly lower than that of control group 7 and 14 days after treatment(P<0.05).The score of wound pain in the two groups was significantly lower than that before dressing removal(P<0.05).The wound pain score after dressing removal was significantly lower than that before dressing r
