NB-UVB联合点阵激光治疗难治性白癜风临床疗效分析

摘　　要：目的:探讨窄谱中波紫外线(NB-UVB)联合点阵激光治疗难治性白癜风患者的疗效及安全性。方法:选取2017年1月-2019年1月在笔者医院就诊的120例难治性白癜风患者纳入研究,选择患者的独立皮损处给予治疗,随机分为两组。实验组:60例,选择NB-UVB联合点阵激光治疗;对照组:60例,给予NB-UVB进行治疗。比较两组治疗后的临床疗效及安全性。结果:6个月治疗结束后,实验组治疗有效率86.67%,高于对照组的66.67%。治疗后1、3、6个月,实验组的患者总体评分(Investigator's global assessment,IGA)明显高于对照组(P<0.05)。治疗后,实验组皮损面积小于对照组,平均复色面积大于对照组(P<0.05);实验组皮损修复时间(1.67±0.27)个月,明显低于对照组的(2.02±0.59)个月,有显著性差异,具有统计学意义(P<0.05)。实验组不良反应发生率低于对照组(P<0.05)。结论:NB-UVB联合点阵激光治疗难治性白癜风可提高患者的治疗有效率和IGA评分,治疗过程安全且不良反应少。Objective To explore curative effect and safety of narrow-band ultraviolet B(NB-UVB)combined with fractional laser on refractory vitiligo.Methods During the period from January 2017 to January 2019,120 patients with refractory vitiligo who were treated in the hospital were enrolled.The patients were treated at independent skin lesion sites.60 cases in experimental group were treated with NB-UVB combined with fractional laser,while 60 cases in control group were treated with NB-UVB.The clinical curative effect and safety after treatment were compared between the two groups.Results At 6 months after treatment,response rate of treatment in experimental group was higher than that in control group(86.67%vs 66.67%).At 1 month,3 months and 6 months after treatment,scores of investigator's global assessment(IGA)in experimental group were significantly higher than those in control group(P<0.05).After treatment,skin lesion area of experimental group was smaller than that of control group,while average complex colour area was larger than that of control group(P<0.05).The skin lesion repair time of experimental group was significantly shorter than that of control group[(1.67±0.27)months vs(2.02±0.59)months](P<0.05).The incidence of adverse reactions such as redness and oozing in experimental group was lower than that in control group(P<0.05).Conclusion NB-UVB combined with fractional laser in treatment of refractory vitiligo can improve response rate of treatment and IGA score.It is safe and there are few adverse reactions in treatment process.
