窄谱中波紫外线联合凉血消银汤治疗银屑病疗效观察及对血清VEGF的影响研究

摘　　要：目的:评价窄谱中波紫外线联合凉血消银汤治疗银屑病的临床疗效以及对血清血管内皮生长因子表达的影响。方法:选取2016年1月-2018年12月笔者科室收治的83例银屑病患者为研究对象,根据患者治疗方案进行分组,其中采用凉血消银汤治疗的38例患者为对照组,采用窄谱中波紫外线联合凉血消银汤进行治疗的45例患者为观察组,比较两组治疗前后的临床疗效以及血清VEGF表达水平。结果:观察组疗效明显高于对照组(P<0.05)。两组治疗后PASI评分、中医症状评分显著低于治疗前(P<0.05),血清VFGF表达水平明显低于治疗前(P<0.05),治疗后观察组PASI评分、中医症状评分、血清中VFGF表达水平均显著低于对照组(P<0.05)。血清VFGF表达水平与PASI评分呈正相关(P<0.05)。结论:窄谱中波紫外线联合凉血消银汤治疗银屑病能够有效提高临床疗效,可能与降低血清VEGF表达水平有关。Objective To analyze the clinical efficacy of narrow-band UVB combined with Liangxue Xiaoyin Decoction in patients with psoriasis and its effects on serum vascular endothelial growth factor(VEGF)expression.Methods Eighty-three patients admitted to our department from January 2016 to December 2018 were enrolled as subjects.Patients were grouped according to their treatment regimen.38 patients treated with Liangxue Xiaoyin Decoction treated as the control group,45 patients treated with narrow-band UVB combined with Liangxue Xiaoyin Decoction were the observation group.The clinical efficacy before and after treatment and the serum VEGF expression were compared between the two groups.Results The total clinical efficacy of the psoriasis patients in the observation group was significantly higher than that in the controlgroup (P <0.05). The pre-treatment PASI scores of the two groups of psoriasis patients were significantly higher thanthose after treatment (P <0.05),the PASI scores of the psoriasis patients in the observation group were significantly lower thanthose in the control group (P <0.05). The scores of TCM symptoms before treatment in the two groups were significantly higherthan those after treatment (P <0.05). The scores of TCM symptoms in the observation group were significantly lower than thosein the control group (P <0.05). The expressions of VFGF in serum of patients with psoriasis before treatment were significantlyhigher than those after treatment (P <0.05). The expressions of VFGF in serum of patients with psoriasis in the observationgroup was significantly lower than that in the control group (P <0.05). According to the correlation analysis, the expression ofVFGF in serum was positively correlated with PASI score (P <0.05). Conclusion Narrow-band UVB combined with LiangxueXiaoyin Decoction can effectively improve the clinical efficacy, and reduce the neovascularization of skin lesions by reducingthe serum VEGE expression level in patients with psoriasis.
