摘 要:目的观察超声引导下针刀治疗网球肘的疗效。方法60例受试者按计算机生成随机数字表分为超声引导针刀组、对照组各30例。超声引导针刀组采用超声引导下针刀松解术治疗,对照组采用氟比洛芬贴布外敷在最疼痛局部治疗。治疗后14 d、30 d分别进行1次随访,对于受试者治疗效果及超声影像疗效进行评价。结果2组受试者在经过治疗后治疗效果均有所改善,2组患者疼痛视觉模拟评分(VAS)较治疗前均有明显下降。治疗后14 d、30 d,超声引导针刀组VAS疼痛评分均显著低于对照组(P<0.05)。治疗后超声引导针刀组伸肌总腱厚度增加程度明显改善,且明显优于对照组(P<0.01);局部组织血流信号强度情况超声引导针刀组改善明显,明显优于对照组(P<0.01)。结论在高频超声引导下针刀治疗网球肘有显著的疗效。高频超声可以清晰辨识病变位置,对于需要松解伸肌总腱处进行精确定位,明显改善了伸肌总腱厚度以及局部组织血肿情况。可作为临床上精确安全治疗网球肘的一种有效方法。Objective To observe the effect of ultrasonic guided acupotomology on tennis elbow.Methods Sixty subjects were randomly divided into ultrasonic guided acupotomology group and control group,30 cases in each group.The ultrasound-guided acupotomology group was treated with ultrasound-guided acupotomysis,while the control group was treated with flurbiprofen sticking cloth applied externally to the most painful local treatment.A follow-up visit was conducted 14 days and 30 days after treatment to evaluate the therapeutic effect and ultrasonic imaging effect of the subjects.Results After treatment,the treatment effect of the two groups of subjects improved,and VAS pain score of the two groups decreased significantly compared with that before treatment.VAS pain scores in the ultrasound-guided acupotomology group were significantly lower than those in the control group at 14 and 30 days after treatment(P<0.05).Through ultrasonic measurement,it was found that after treatment,the increase of the thickness of the extensor wrist tendon in the ultrasound-guided acupotomology group was significantly improved,which was significantly better than the control group(P<0.01).The intensity of local tissue blood flow signal was significantly improved in the ultrasound-guided acupotomology group,which was significantly better than the control group(P<0.01).Conclusion Under the guidance of high frequency ultrasound technique,acupotomology is effective in the treatment of tennis elbow.High-frequency ultrasound can clearly identify the location of lesions and accurately locate the common extensor tendon,which needs to be released,significantly improving the thickness of the common extensor tendon and local tissue hematoma.It can be used as an effective method to treat external humeral epicondylitis accurately and safely.
