摘 要:目的探讨早期肾损伤标志物在老年高血压、糖尿病肾损害诊断中的价值。方法将本院2016年10月至2018年10月收治的患有易引起肾脏损害疾病的60例患者纳入研究,根据引发肾损害疾病的不同,分为高血压组28例和糖尿病组32例,另选取同期在本院体检中心进行检查的30名健康志愿者作为对照组,所有研究对象均行尿肾功联合检测,比较3组的血清半胱氨酸蛋白酶抑制剂C(Cys-C)、β2-微球蛋白(β2-MG)、尿微量白蛋白(mALB)、尿素氮(BUN)以及血肌酐(Scr)的水平差异。结果3组的BUN、Scr水平差异无统计学意义(P>0.05),但高血压组和糖尿病组患者的Cys-C、β2-MG以及mALB水平均显著高于对照组(P<0.05);与对照组相比,高血压组和糖尿病组的Cys-C、β2-MG、mALB检测阳性率均明显较高(P<0.05)。结论Cys-C、β2-MG、mALB作为早期肾损伤标志物,其水平较健康人明显上升,在老年高血压、糖尿病患者肾损害的诊断中检出率较高,值得推广和使用。Objective To investigate the value of early renal injury markers in the diagnosis of hypertensive and diabetic kidney damage in the elderly.Methods Included 60 patients with conditions prone to kidney damage admitted to our hospital between October 2016 and October 2018.According to etiology of the kidney damage,the patients were divided into the hypertension group(n=28)and the diabetes group(n=32).A contemporary cohort of 30 health checkup volunteers in our hospital were assigned to the control group.All the subjects underwent urinalysis and renal function tests.The levels of serum Cystatin C(Cys-C)andβ2-microglobulin(β2-MG),urinary microalbumin(mALB),blood urea nitrogen(BUN),and serum creatinine(Scr)were compared among the three groups.Results There were no significant differences in BUN and Scr levels among the three groups(P>0.05),but the levels of Cys-C,β2-MG and mALB in the hypertension group and the diabetic group were significantly higher than those in the control group(P<0.05).Compared with the control group,the positive rates of Cys-C,β2-MG and mALB in the hypertension group and the diabetic group were significantly higher(P<0.05).Conclusion Cys-C,β2-MG and mALB as markers of early kidney injury are significantly elevated and frequently detected in elderly patients with hypertensive and diabetic kidney damage compared with normal individuals.These findings justify their widespread use in clinical settings.
