口服补铁对慢性心衰伴缺铁患者心功能和生活质量的影响

摘　　要：目的观察口服补铁对慢性心衰伴缺铁患者心功能和生活质量的影响。方法将100例慢性心衰伴缺铁患者随机分为对照组和多糖铁组,对照组给予常规抗心衰治疗,多糖铁组在常规抗心衰治疗上给予日1次口服多糖铁复合物胶囊150 mg,共24 w,观察两组治疗前,后4、8、12、16、20、24 w运动能力、心功能情况、生活质量、左室射血分数、血浆N末端B型脑钠肽原(NT-pro-BNP)和血清铁蛋白的变化。结果与对照组比较,多糖铁组患者NYHA心功能分级改善、6 min步行试验距离增加、左室射血分数提高及血浆NT-pro-BNP水平降低均优于对照组(P<0.05),多糖铁组治疗后生活质量评分、血清铁蛋白水平改善更加显著(P<0.05)。结论口服补铁能够有效纠正缺铁状态,显著改善慢性心衰患者的心功能和生活质量,值得推广应用。Objective To observe the effect of oral iron supplementation on the cardiac function and life quality of the patients with chronic heart failure and iron deficiency.Methods 100 patients with chronic heart failure and iron deficiency were randomly divided into control group and polysaccharide iron group.The control group was given conventional treatment for heart failure,while the polysaccharide iron group was given 150 mg of oral polysaccharide iron complex capsules once a day on the basis of routine treatment for 24 weeks.The changes of exercise capacity,cardiac function,quality of life,left ventricular ejection fraction(LVEF),NT-pro-BNP and serum ferritin were observed before and 4,8,12,16,20 and 24 weeks after treatment.Results In the polysaccharide iron group,NYHA cardiac function grading improved,6-minute walk test(6MWT)distance increased,left ventricular ejection fraction increased and level of plasma NT-pro-BNP decreased,all of which were better than those in the control group(P<0.05).The score of life quality and level of serum ferritin after treatment in the polysaccharide iron group improved more significantly(P<0.05).Conclusion Oral iron supplementation can effectively remedy iron deficiency and significantly improve the cardiac function and life quality of the patients with chronic heart failure,which is worthy of popularization and application.
