摘 要:目的探究分析在妊娠期糖尿病产程中采用分娩镇痛对产时内分泌及新生儿血糖造成的影响。方法选取该院于2018年6月—2020年3月收治的280例妊娠期糖尿病患者为该次研究资料来源,按照随机分类法将其随机划分为观察组和参照组,各140例,观察组患者采取腰硬联合麻醉分娩镇痛,参照组患者未采取腰硬联合麻醉分娩镇痛,观察对比两组患者产时内分泌与新生儿的血糖情况。结果相较于参照组患者,观察组患者的活跃期时间更短(P<0.05),但潜伏期和第二产程时间比较两组患者差异无统计学意义(P>0.05);T0阶段的两组患者血糖水平差异无统计学意义(P>0.05),观察组患者在T1~T3阶段的血糖水平和T4阶段的皮质醇水平均更低(P<0.05);观察组的新生儿血糖水平更高,脐血皮质醇则显著更低(P<0.05)。结论在妊娠期糖尿病产程中采用腰硬联合麻醉分娩镇痛可以有效缩短分娩时的活跃期时长,能够起到改善产妇本身以及新生儿的血糖和皮质醇水平的作用,具有较高的临床应用价值。Objective To explore and analyze the effects of labor analgesia on endocrine and neonatal blood glucose during labor in gestational diabetes. Methods 280 cases of gestational diabetes patients admitted to the hospital from June 2018 to March 2020 were selected as the data source for this study. According to the random classification method, they were randomly divided into observation group and reference group with 140 cases each, observation group received combined spinal-epidural anesthesia for labor analgesia, and the reference group did not use combined spinal-epidural anesthesia for labor analgesia. Observe and compare the endocrine and neonatal blood glucose of the two groups during labor. Results Compared with the reference group, the observation group had a shorter active period(P<0.05), but there was no statistically significant difference between the incubation period and the time of the second stage of labor(P>0.05);the blood glucose of the two groups in the T0 stage, there was no statistically significant difference in the level(P>0.05). The blood glucose level in the T1-T3 stage and the cortisol level in the T4 stage of the observation group were lower(P<0.05);the neonatal blood glucose level in the observation group was higher and the cord blood cortex Alcohol was significantly lower(P<0.05).Conclusion The use of combined spinal-epidural anesthesia for labor analgesia in the labor process of gestational diabetes can effectively shorten the active period of labor, and can improve the blood glucose and cortisol levels of the mother and newborns, and has high clinical application value.
