摘 要:目的:探讨北京市各级医院医务人员对双向转诊认知及实施情况。方法:选取2所三级医院、3所二级医院和5家社区卫生服务中心,采用目的抽样的方法,对医务人员进行问卷调查。结果:63%的三级医院医务人员对双向转诊比较了解,社区及二级医院认知度有待提高,比较了解分别占37%、35%;双向转诊制度实施中,各级医院医务人员对双向转诊制度的认可度及满意度有待进一步提高;其中,二三级医院医务人员认可度及满意度较高,社区较低;三级医院接诊上转患者数量最多,社区医院接诊下转患者数量最少,“上转容易下转难”的问题仍然有待进一步解决。3个不同级别医院医务人员对双向转诊制度的认知度、认可度及满意度的差异均具有统计学意义(P<0.05)。结论:加强政策引导,加大宣传力度;制定统一标准,简化转诊流程;推进医联体建设,加强医院沟通;建立激励与约束机制,提高医务人员积极性。Objective:To explore the cognition and implementation of two-way referral among medical staff in hospitals at all levels in Beijing.Methods:Select 2 tertiary hospitals,3 secondary hospitals and 5 community health service centers,and use the method of purpose sampling to conduct a questionnaire survey of medical staff.Results:63%of medical staff in tertiary hospitals have a better understanding of two-way referrals.The awareness of community and second-level hospitals needs to be improved,accounting for 37%and 35%respectively;in the implementation of the two-way referral system,medical staff at all levels of hospitals The recognition and satisfaction of the two-way referral system needs to be further improved;among them,the recognition and satisfaction of medical staff in second-and third-level hospitals are relatively high,but the community is relatively low;third-level hospitals receive the largest number of referral patients,and community hospitals receive treatment The number of patients with downward transfer is the smallest,and the problem of"upward transfer is easy and downward transfer is difficult"still needs to be further resolved.The differences in the cognition,recognition and satisfaction of the two-way referral system among the medical staff of the three different levels of hospitals were statistically significant(P<0.05).Conclusion:Strengthen policy guidance and increase publicity;formulate uniform standards and simplify the referral process;promote the construction of medical consortia and strengthen hospital communication;establish incentive and restraint mechanisms to increase the enthusiasm of medical staff.
