摘　　要：目的收集腹腔镜前列腺癌根治术前患者盆腔磁共振成像(MRI)资料,寻找对术后尿失禁具有预测价值的盆底形态学指标。方法回顾性分析2017—2019年于郑州大学第一附属医院接受腹腔镜前列腺癌根治术治疗的57例患者的临床资料,根据术后3个月尿控情况分为尿失禁组(32例)和尿控组(25例)。在患者术前盆腔MRI图像上测量前列腺体积、膜性尿道长度(MUL)、耻骨直肠肌(PRM)厚度、膜性尿道角度(AMU)、膀胱颈(BN)到耻骨尾骨线(PCL)的垂直距离、BN与耻骨的最短距离、尿道球部(BU)到PCL的垂直距离、BU与耻骨的最短距离,比较两组上述指标大小。结果尿失禁组和尿控组MUL分别为(8.51±2.35)、(12.75±4.41)mm,尿失禁组MUL短于尿控组,差异有统计学意义(P<0.05)。两组余MRI指标差异无统计学意义(P>0.05)。结论术前MUL较长的患者腹腔镜前列腺癌根治术后尿控功能更好。Objective To collect patients’pelvic magnetic resonance imaging(MRI)data before laparoscopic radical prostatectomy,and to find pelvic floor morphological indicators that have predictive value for postoperative urinary incontinence.Methods The clinical data of 57 patients who underwent laparoscopic radical prostatectomy in the First Affiliated Hospital of Zhengzhou University from 2017 to 2019 were retrospectively analyzed.According to the urinary control status 3 months after the operation,they were divided into urinary incontinence group(32 cases)and urinary control group(25 cases).The following data were measured on the preoperative pelvic MRI:prostate volume,membranous urethra length(MUL),thickness of puborectalis muscle(PRM),angle of membranous urethra(AMU),vertical distance from bladder neck(BN)to pubic coccyx line(PCL),the shortest distance between BN and pubic bone,vertical distance from bulbous urethra(BU)to PCL,and the shortest distance between BU and pubic bone.The above indicators were compared between the two groups.Results MUL of urinary incontinence group and urinary control group were(8.51±2.35)mm and(12.75±4.41)mm,respectively.MUL of urinary incontinence group was shorter than that of urinary control group(P<0.05),and there was no statistical difference in the remaining MRI indexes between the two groups(P>0.05).Conclusion Patients with longer MUL preoperatively have better urinary control after laparoscopic radical prostatectomy.
