桉柠蒎联合阿奇霉素对儿童慢性鼻-鼻窦炎的治疗效果及对免疫球蛋白E、Toll样受体4水平的影响

摘　　要：目的:探讨桉柠蒎联合阿奇霉素对儿童慢性鼻-鼻窦炎的治疗效果及对免疫球蛋白E(IgE)、Toll样受体4(TLR4)水平的影响。方法:选取2017年3月至2019年3月在沧州市中心医院就诊的慢性鼻-鼻窦炎患儿96例,按照随机数字表法分为联合组和阿奇霉素组,每组48例。阿奇霉素组患儿给予阿奇霉素颗粒治疗,联合组患儿在阿奇霉素组的基础上给予桉柠蒎肠溶软胶囊治疗。检测两组患儿的IgE、TLR4水平,评估临床症状、疼痛及Lund-Kennedy鼻内镜评分,统计临床疗效、复发情况及不良反应发生情况。结果:联合组患儿治疗后的IgE、TLR4水平明显低于阿奇霉素组同期,差异均有统计学意义(P<0.05)。联合组患儿治疗后的临床症状(鼻部、睡眠障碍、情感、相关症状及体征)评分明显低于阿奇霉素组同期,差异均有统计学意义(P<0.05)。联合组患儿治疗后的疼痛评分、Lund-Kennedy鼻内镜评分明显低于阿奇霉素组同期,差异均有统计学意义(P<0.05)。联合组患儿的总有效率明显高于阿奇霉素组,复发率明显低于阿奇霉素组,差异均有统计学意义(P<0.05)。结论:桉柠蒎联合阿奇霉素对儿童慢性鼻-鼻窦炎的治疗效果显著,能有效降低患儿IgE、TLR4水平,改善患儿临床症状,缓解其痛苦,促进患儿恢复。OBJECTIVE:To probe into the efficacy of eucalyptol limonene and pinene combined with azithromycin in the treatment of chronic rhinosinusitis in children and its effect on immune globulin E(IgE)and Toll-like receptor-4(TLR4)levels.METHODS:Totally 96 children with chronic rhinosinusitis admitted into our hospital from Mar.2017 to Mar.2019 were selected and divided into combined group and azithromycin group via random number table,with 48 cases in each group.The azithromycin group was treated with Azithromycin granules,the combined group was given Eucalyptol limonene and pinene enteric-coated soft capsule based on the azithromycin group.The IgE and TLR4 levels of both groups were detected,scores of clinical symptom,pain and Lund-Kennedy nasal endoscopy scores were evaluated,clinical efficacy,incidences of recurrence and adverse drug reactions were collected.RESULTS:The IgE and TLR4 levels of combined group after treatment were significantly lower than those of azithromycin group,with statistically significant differences(P<0.05).Symptom(nose,sleep disorders,emotion,related symptoms and sign)scores of combined group after treatment were significantly lower than those of azithromycin group,with statistically significant differences(P<0.05).The pain score and Lund-Kennedy nasal endoscopy score of combined group after treatment were significantly lower than those of azithromycin group,with statistically significant differences(P<0.05).The total effective rate of combined group was significantly higher than azithromycin group,while the incidence of recurrence was significantly lower,with statistically significant differences(P<0.05).CONCLUSIONS:The efficacy of eucalyptol limonene and pinene combined with azithromycin in the treatment of chronic rhinosinusitis in children is remarkable,which can significantly reduce IgE and TLR4 levels,improve clinical symptoms,relieve pain and promote recovery.
