摘 要:目的:探讨黄连、黄芩对鲍曼不动杆菌的体外抑制作用,为临床用药提供指导。方法:采用微量肉汤稀释法,观察黄连、黄芩、盐酸小檗碱、黄芩苷单用及其联合替加环素对鲍曼不动杆菌的体外抑制作用。黄芩、黄连水提物浓度均设置为0.38、0.75、1.50、3.00及6.00 mg/ml(生药浓度);盐酸小檗碱、黄芩苷浓度均设置为5.00、10.00、20.00、40.00及80.00μmol/L;替加环素浓度设置为0.11、0.21、0.43、0.86及1.71μmol/L;鲍曼不动杆菌浓度为(2.00~3.00)×10~5CFU/ml;培养条件为35℃,16 h。结果:抑菌效果随黄连、黄芩、盐酸小檗碱和黄芩苷浓度的增加而增强,且量效关系显著。药物浓度相同时,黄连对鲍曼不动杆菌的抑制效果优于黄芩,盐酸小檗碱对鲍曼不动杆菌的抑制效果优于黄芩苷,差异均有统计学意义(P<0.05)。黄连、黄芩、盐酸小檗碱、黄芩苷与替加环素联合应用可增强替加环素对鲍曼不动杆菌的抑制作用;在一定的替加环素浓度条件下,黄连、黄芩、盐酸小檗碱及黄芩苷分别联合替加环素时,可考虑适度减少替加环素用量。结论:黄连、黄芩、盐酸小檗碱和黄芩苷体外对鲍曼不动杆菌具有一定的抑制作用,其与替加环素联合应用时可考虑适度减少替加环素用量。OBJECTIVE:To probe into the inhibitory effect of coptidis rhizoma and radix scutellariae on Acinetobacter baumannii in vitro,so as to provide guidance for clinical medication.METHODS:Micro broth dilution method was adopted to observe the in vitro inhibitory effects of coptidis rhizoma,radix scutellariae,berberine hydrochloride and baicalin on Acinetobacter baumannii when used alone or in combination with tigecycline.The concentrations of aqueous extract of coptidis rhizoma and radix scutellariae were set as 0.38,0.75,1.50,3.00 and 6.00 mg/ml(concentration of crude drug);the concentrations of aqueous extract of berberine hydrochloride and baicalin were set as 5.00,10.00,20.00,40.00 and 80.00μmol/L;the concentrations of tigecycline were set as 0.11,0.21,0.43,0.86 and 1.71μmol/L;the concentration of A.baumannii was(2.00-3.00)×10~5 CFU/ml;and the culture condition was 35℃,16 h.RESULTS:The antibacterial effect increased with the increase of the concentration of coptidis rhizoma,radix scutellariae,berberine hydrochloride and baicalin,and the dose-effect relationship was significant.At the same drug concentration,the coptidis rhizoma had better inhibitory effect on A.baumannii than that of the radix scutellariae,the berberine hydrochloride had better inhibitory effect on A.baumannii than that of the baicalin,with statistically significant differences(P<0.05).The combination of tigecycline with coptidis rhizoma,radix scutellariae,berberine hydrochloride,baicalin can enhance its inhibitory effect on A.baumannii,at a certain concentration of tigecycline,when coptidis rhizoma,radix scutellariae,berberine hydrochloride and baicalin were for drug combination with tigecycline,the dosage of tigecycline can be appropriately reduced.CONCLUSIONS:Coptidis rhizoma,radix scutellariae,berberine hydrochloride and baicalin have a certain inhibitory effect on A.baumannii in vitro,and their combined use with tigecycline can be considered to moderately reduce the amount of tigecycline.
