摘　　要：目的:了解首都医科大学附属北京友谊医院(以下简称"我院")中药饮片处方书写规范性和合理用药情况,为加强中药饮片处方管理和提高处方质量提供依据。方法:随机抽取我院2019年每月任意2 d的处方,以《处方管理办法》《中药处方格式及书写规范》《中华人民共和国药典》(2015年版)、《医院处方点评管理规范》和《北京市中药饮片调剂规程》等为依据,对处方进行回顾性点评与分析。结果:2019年共抽查中药饮片处方4785张,不合理处方426张,处方不合理率为8.90%;其中,不规范处方160张(占不合理处方的37.56%),包括未按要求标注药物调剂、煎煮等特殊要求和处方中药物未按照君臣佐使顺序排列;用药不适宜处方185张(占不合理处方的43.43%),包括用量不适宜、适应证不适宜、有配伍禁忌或者不良相互作用、药品给药途径不适宜和重复给药;超常处方81张(占不合理处方的19.01%),包括处方金额过大、中药味数过多。结论:我院中医处方的开具还存在一些问题,应重视中药饮片处方的规范性,确保临床用药安全。OBJECTIVE:To investigate the standardization and rational medication of prescriptions of traditional Chinese medicine(TCM)decoction pieces in Beijing Friendship Hospital affiliated to Capital Medical University(hereinafter referred to as"our hospital"),so as to provide a basis for strengthening prescription management and improving prescription quality.METHODS:Prescriptions in our hospital on any two days of each month in 2019 were randomly abstracted and retrospectively reviewed according to the"Prescription Management Measures","Traditional Chinese Medicine Prescription Format and Writing Specifications","Chinese Pharmacopoeia"(2015 Edition),"Hospital Prescription Review Management Practices"and"Regulations for Dispensing Traditional Chinese Medicine Decoction Pieces of Beijing".RESULTS:A total of 4785 prescriptions of TCM decoction pieces were reviewed,426 were irrational prescriptions,with the irrational rate was 8.90%;of which 160 were nonstandard prescriptions(accounted for 37.56%of all the irrational prescriptions),including special requirements such as drug preparations and decoctions that were not marked as required and the drugs in the prescription were not arranged in the order of assistant and guide;185 prescriptions were inappropriate medication(accounted for 43.43%of all the irrational prescriptions),including inappropriate dosage,improper indications,incompatibility or adverse interaction,inaptitude administration route and repeat dosing;81 were abnormal prescriptions(accounted for 19.01%of all the irrational prescriptions),including too large amount of prescription cost and too much categories of TCM.CONCLUSIONS:There are still some problems in TCM prescription in our hospital,which should be paid attention to the standardization of prescriptions of TCM decoction pieces to ensure clinical safe medication.
