血清IL-6联合PCT检验对儿童脓毒症病情的评估

摘　　要：目的探讨血清白介素-6(IL-6)联合降钙素原(PCT)检验对感染相关儿童脓毒症患儿病情评估的价值。方法选取2018年12月至2019年12月在内蒙古医科大学附属医院儿科诊治疑似感染相关儿童脓毒症100例患儿资料,经快速序贯器官衰竭评估(qSOFA)评分≥2分,将其设为研究组;选择同期健康儿童60例资料设为对照组。两组均采用血清IL-6联合PCT检验,分析其与儿童脓毒症病情及预后的相关性。结果研究组的血清IL-6和PCT水平均高于对照组,差异均有统计意义(t值分别为6.9129、8.0120,均P<0.01)。研究组的血清IL-6和PCT敏感度及特异度均较高,但差异均无统计学意义(χ^2值分别为1.8706、1.9762,均P<0.05)。研究组中的脓毒症组血清IL-6和PCT水平均高于非脓毒症组,差异均有统计学意义(t值分别为6.3914、7.0795,均P<0.01);脓毒症组中的预后良好组血清IL-6和PCT水平均低于预后不良组,差异均有统计学意义(t值分别为3.6743、8.4279,均P<0.01)。结论在疑似感染相关儿童脓毒症患儿中应用血清IL-6联合PCT检验,可诊断是否感染脓毒症,其敏感度、特异度均较高,为临床治疗及预后效果提供依据,有一定的临床应用价值。Objective To analyze value of serum interleukin-6(IL-6)and procalcitonin(PCT)combined detection in evaluation of disease condition of infection-associated sepsis in children.Methods The clinical data of 100 children who were diagnosed as suspected infection-associated sepsis and treated in our hospital from December 2018 to December 2019 were collected.The children with score in quick Sequential Organ Failure Assessment(qSOFA)≥2 points were enrolled in the study group,and another 60 healthy children during the same period were selected as the controls.Serum levels of IL-6 and PCT of all children in the two groups were detected.The correlations of serum levels of IL-6 and PC with disease condition and prognosis of sepsis in children were analyzed.Results The serum levels of IL-6 and PCT of the children in the study group were both higher than those of the children in the control group,and the differences were statistically significant(t=6.9129,t=8.0120,both P<0.01).The sensitivity and specificity of serum IL-6 and PCT in the study group were both higher,but there were no differences between IL-6 and PCT(χ^2=1.8706 and 1.9762 respectively,both P>0.05).The serum IL-6 and PCT levels of the children in sepsis subgroup in the study group were both higher than those of the children in non-sepsis subgroup in the study group and there were significant differences between the two subgroups(t=6.3914,t=7.0795,both P<0.01).In sepsis subgroup,the serum levels of IL-6 and PCT in good prognosis subgroup were both lower than those in the poor prognosis subgroup,and there were significant differences between the two subgroups(t=3.6743,t=8.4279,both P<0.01).Conclusion For those children with suspected infection-associated sepsis,the combined detection of serum IL-6 and PCT levels can determine the diagnosis of infection-associated sepsis in children,and it can provide some basis for clinical treatment and judgement of prognosis of the disease due to their higher sensitivity and specificity,therefore,the combined detection is of
