摘  要:目的分析高压氧联合脑苷肌肽治疗对创伤性颅脑损伤患者血清神经元特异性烯醇化酶(NSE)、可溶性凋亡相关因子(sFas)蛋白的影响,同时评价患者的早期临床结果。方法回顾性分析86例创伤性颅脑损伤患者,根据药物使用情况将患者分为对照组42例和治疗组44例。2组患者均采用降颅压、抗感染、保护脑细胞、解痉挛等常规治疗。抽取患者入院时、治疗2 d、治疗7 d、治疗14 d时的静脉血,ELISA法测定血清中NSE、sFas蛋白的含量。同时,比较2组患者术后恢复情况,评价不同治疗方案的疗效。结果治疗组患者血清中NSE、sFas蛋白与对照组在治疗的不同时间点比较显著降低,差异有统计学意义(P<0.05)。治疗组GCS评分改善及住院时间优于对照组,GCS评分改善更大、住院时间更短,差异有统计学意义(P<0.05)。结论对于创伤性颅脑损伤患者,治疗中采用高压氧联合脑苷肌肽的方法可取得更好的早期治疗结局,其机制与治疗方法抑制神经细胞凋亡等细胞死亡有关。Objective To investigate the effects of hyperbaric oxygen(HBOT)combined with cattle encephalon glycoside in treatment of traumatic brain injury(TBI)on the serum levels of NSE and sFas protein.Methods The clinical data about 86 patients with TBI were retrospectively analyzed,who were were divided into observation group and control group according to drug medication.The patients in both groups in both groups were treated by conventional therapy including the measures of reducing intracranial pressure,anti-infection,protecting brain nerve cells and relieving spasmolysis.Moreover the patients in control group were treated by cattle encephalon glycoside,however,the patients in observation group were treated by HBOT combined with cattle encephalon glycoside.The clinical curative effects and recovery status of patients were observed and compared between the two groups.In addition the serum levels of NSE and sFas protein in both groups were detected by ELISA.Results The serum levels of NSE and sFas protein in observation group in different time points were significantly lower than those in control group(P<0.05).Moreover the GCS scores and the length of staying in hospital in observation group were superior to those in control group(P<0.05).Conclusion The HBOT combined with cattle encephalon glycoside in treatment of TBI can obtain better early therapeutic effects,and its action mechanism may be related to the inhibition of nerve cells’deaths by including the cell apoptosis.
