饮食指导联合PRECEDE模式护理对慢性阻塞性肺炎患者肺功能及血清蛋白的影响

摘　　要：目的研究饮食指导联合PRECEDE模式护理对慢性阻塞性肺炎(COPD)患者肺功能及血清蛋白的影响。方法以2015年3月至2016年3月治疗的100例COPD患者为研究对象,随机分为对照组和观察组,每组50例。2组患者均行常规护理,对照组患者在此基础上进行饮食指导,而观察组患者则采用饮食指导和PRECEDE联合的护理模式,护理前以及护理2个月后,采用圣乔治呼吸问卷(SGRQ)量表评价2组患者生活质量,检测2组患者肺功能指标和血清蛋白指标,具体包含肺活量(VC)、用力肺活量(FVC)和1 s用力呼气量(FEV1)、血清蛋白(ALB)、前血清蛋白(PA)等,评价2组患者遵医嘱情况。结果护理2个月后,SGRQ评分明显下降(P<0.05),VC、FVC、FEV1、FEV1/FVC、ALB和PA水平显著上升(P<0.05);其中观察组患者SGRQ评分明显低于对照组(P<0.05),肺功能和血清蛋白各指标均显著高于对照组(P<0.05),同时,护理后饮食、运动、用药和复查的依从率均显著高于对照组,2组差异有统计学意义(P<0.05)。结论将饮食指导联合PRECEDE护理模式应用于COPD患者对于临床护理有重要应用价值,能显著改善患者肺功能和营养状态,有利于提高患者生活质量和治疗依从性。Objective To investigate the effects of dietary guidance combined with PRECEDE nursing mode on the lung function and serum protein of patients with chronic obstructive pulmonary disease(COPD).Methods 100 patients with COPD who were treated in our hospital from March 2015 to March 2016 were randomly divided into control group and observation group,with 50 cases in each group.The patients in both groups were treated by routine nursing intervention,on that basis,the patients in control group given dietary guidance,however,the patients in observation group were treated by diet guidance combined with PRECEDE nursing mode.Before the nursing intervention and at 2 months after the nursing,the St.George’s Respiratory Questionnaire(SGRQ)scale was used to evaluate the life quality of patients,and the lung function indexws and serum protein indexes were detected in both groups,including vital capacity(VC)and forced vital capacity(FVC)and 1s forced expiratory volume(FEV1),serum protein(ALB),pre-serum protein(PA).Moreover the compliance with the doctor’s advice was observed and compared between the two groups.Results After 2-month nursing intervention,the scores of the indexes were significantly improved in both groups,however,the SGRQ scores were significantly decreased(P<0.05),and the levels of VC,FVC,FEV1 and FEV1/FVC as well as the levels of ALB and PA were increased significantly(P<0.05).The SGRQ scores in observation group were significantly lower than those in control group,however,the lung function and serum protein levels were significantly higher than those in control group(P<0.05).Moreover,the compliance rates of diet,exercise,medication and review after treatment in observation group were significantly higher than those in control group(P<0.05).Conclusion The application of dietary guidance combined with PRECEDE nursing mode for patientts with COPD has important significance in clinical nursing,which can significantly improve the lung function and nutritional status of patients,which is conducive to improving t
