针刺联合半夏厚朴汤加减治疗痰气郁结型胃食管反流病疗效观察  

Effect of Acupuncture Combined with Banxia Houpo Decoction for Gastroesophageal Reflux Disease of Intermingled Phlegm and Qi Type

摘　　要：目的:观察针刺联合半夏厚朴汤加减治疗痰气郁结型胃食管反流病临床疗效。方法:将80例胃食管反流病患者采用随机数字表法分为对照组和观察组各40例。对照组予常规西药治疗,观察组予针刺联合半夏厚朴汤加减治疗,疗程1个月。治疗前后评定反流性疾病问卷(RDQ)评分、胃镜检查积分、胃食管反流病生活质量量表评分(GERD-HRQL);于治疗前后测定患者血清胃促生长素及胃泌素水平;并于治疗后判定其疗效。结果:观察组总有效率明显优于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组RDQ评分、胃镜检查积分及GERD-HRQL评分均改善,且明显低于对照组,差异具有统计学意义(P<0.05)。观察组患者治疗后血清胃促生长素及胃泌素水平均升高,且明显高于对照组,差异具有统计学意义(P<0.05)。结论:针刺联合半夏厚朴汤加减治疗痰气郁结型胃食管反流病疗效显著,可明显改善患者胃食管反流临床症状,减轻食管黏膜炎性反应,提高生活质量,这可能与其调节胃促生长素及胃泌素水平相关。Objective:To observe the clinical effect of acupuncture combined with Banxia Houpo Decoction(半夏厚朴汤)for gastroesophageal reflux disease(GERD)of intermingled phlegm and qi type.Methods:Eighty patients with(GERD)were randomly divided into control group and treatment group,40 cases in each group.The control group was given conventional western medicine,while the treatment group was given acupuncture combined with Banxia Houpo Decoction.Both groups were treated for 1 month.Reflux diagnostic questionnaire(RDQ),score of gastroscope examination and GERD-health related quality of life(GERD-HRQL)were assessed before and after treatment.The serum levels of ghrelin and gastrin were measured before and after treatment.The curative effect was evaluated after treatment.Results:The effective rate in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).After the treatment,RDQ,scores of gastroscope examination and GERD-HRQL in the treatment group were decreased,and were lower than those in control group,with statistical difference(P<0.05).After the treatment,the serum levels of ghrelin and gastrin were increased in the treatment group and was higher than those in the control group,with statistical difference(P<0.05).Conclusions:Acupuncture combined with Banxia Houpo Decoction for GERD of intermingled phlegm and qi type is of obviously effect.It could relieve clinical symptoms,reduce esophageal mucosa inflammation and improve life quality,which might be related to the regulation of serum levels of ghrelin and gastrin.
