摘 要:目的探究观察经皮胆红素测定与血清胆红素测定在新生儿黄疸诊断中的差异。方法本次研究的起始终止时间分别设定为2017年12月与2019年12月,将105名足月新生儿黄疸患者纳入研究,对其经皮胆红素(TCB)与血清胆红素(TSB)水平进行测定,再对所有患者的TCB与TSB进行相关性分析。结果新生儿黄疸患者的经皮胆红素水平对比血清总胆红素及血清间接胆红素,结果无统计学差异(P>0.05);新生儿黄疸患者经皮胆红素、血清总胆红素及血清间接胆红素两两对比结果均呈线性相关(P=0.0000);患者黄疸阳性预测值、阴性预测值随着总胆红素浓度的升高也表现出升高的趋势,并且当总胆红素浓度处于60.1 umol/L时,黄疸阳性预测值、阴性预测值、特异度均高于其他浓度水平。(P<0.05)。结论经皮胆红素测定与血清胆红素测定在新生儿黄疸诊断中具有高度相关性,但是经皮胆红素测定具有高效、快捷、无创、简单以及动态监测的优势,将其作为新生儿胆红素水平的监测手段价值显著。Objective To explore the differences between the determination of percutaneous bilirubin and serum bilirubin in the diagnosis of neonatal jaundice.Methods The start and end time of this study were set in December 2017 and December 2019,respectively.105 full-term neonatal jaundice patients were included in the study,and their percutaneous bilirubin(TCB)and serum bilirubin were included in the study.(TSB)levels were measured,and correlation analysis was performed on TCB and TSB in all patients.Results Percutaneous bilirubin levels in neonatal jaundice patients were compared with total bilirubin and indirect bilirubin in serum,and there were no statistical differences(P>0.05).The pairwise comparison of percutaneous bilirubin,serum total and indirect bilirubin showed a linear correlation(P=0.0000);The positive and negative predictive values of jaundice also showed an increasing trend with the increase of total bilirubin concentration,and when the total bilirubin concentration was 60.1 umol/L,the positive predictive value,negative predictive value,and specificity of jaundice were higher than other values.(P<0.05).Conclusions Concentration of percutaneous bilirubin and serum bilirubin both are highly relevant in the diagnosis of neonatal jaundice,and determination of percutaneous bilirubin has the advantages of high efficiency,fastness,non-invasiveness,simplicity,and dynamic monitoring.So it has significant value at monitoring of bilirubin levels in children.
