摘　　要：目的探讨保留浅静脉的指掌侧V-Y推进皮瓣修复指端皮肤软组织缺损的临床疗效。方法2016年1月-2018年11月,收治19例(21指)指端皮肤软组织及骨缺损患者,均应用保留浅静脉的指掌侧V-Y推进皮瓣予以修复,术后随访评估皮瓣外观、感觉恢复及患指屈伸活动度有无丧失。结果21例皮瓣均成活,随访时间1~17个月,皮瓣质地好,外形佳,感觉恢复满意,两点辨别觉为3~5 mm,平均4 mm,患指屈伸活动度较术前无明显减小。结论保留浅静脉的指掌侧V-Y推进皮瓣修复指端组织缺损能改善皮瓣静脉回流,皮瓣拥有良好的感觉、外观,适宜临床推广。Objective To investigate the method and clinical effect of volar V-Y advancement flap with preservation of superficial vein.Methods From Jan.2016 to Nov.2018,nineteen patients with twentyone fingertip defects underwent reconstruction with volar V-Y advancement flap with preservation of superficial vein.Appearance and sensation of the flap were assessed in follow-up,as well as ranges of motion of the wound fingers.Results All 21 flaps survived.Postoperative follow-up ranged from 1 to17 months.The flaps have good texture,satisfactory sensation recovery,and good appearance.The twopoint discrimination ranged from 3 mm to 5 mm,with an average of 4 mm.There were no significant change in rang of motion of the wounded fingers.Conclusion Preservation of superficial vein of volar V-Y advancement flap can improve venous drainage of the flap.Volar V-Y advancement flap with preservation of superficial vein is a reliable option to reconstruct fingertip defect,with good sensation and satisfactory appearance.
