Th17/Treg和Th1/Th2细胞失衡对变应性鼻炎发病机制的研究

摘　　要：目的通过检测变应性鼻炎(AR)患者外周血CD4+CD25+CD127-调节性T细胞(Treg)水平,Th17、Th1和Th2相关细胞因子血清学水平,初步探讨Th17/Treg细胞、Th1/Th2细胞相关细胞因子及其相互关系在变应性鼻炎发病机制中的作用,为变应性鼻炎的诊断和治疗提供理论和实验依据。方法纳入30例变应性鼻炎患者为实验组,20例健康体检儿童为对照组。采用流式细胞术(FCM)检测外周血CD4+CD25+CD127-Treg细胞水平,采用流式细胞微球阵列术(CBA)检测Th17、Th1、Th2细胞相关细胞因子的血清水平。结果AR组CD4+CD25+CD127-Treg细胞含量明显低于正常人(P<0.001)。AR患者组Th2相关细胞因子IL-6、IL-10血清水平显著升高(P<0.05),IL-17血清水平明显低于正常人(P<0.05)。患者组中,IL-2与TNF-α(r=0.665,P<0.001)、IL-4(r=0.440,P<0.05)呈正相关关系;IFN-γ与IL-10呈正相关关系(r=0.568,P<0.05)。结论Th17细胞与Treg细胞存在显著失衡,Th17/Treg和Th1/Th2细胞及其分泌的细胞因子相互作用在变应性鼻炎的发病机制及发展中起重要作用。Objective The purpose of this study is to preliminarily explore Th17/Treg cells,Th1/Th2 related cytokines and the role of their mutual relationship in the pathogenesis of AR by detecting the levels of CD4+CD25+CD127-Treg in peripheral blood and the serum levels of Th1,Th2,Th17 related cytokines of AR,and then to provide theoretical and experimental basis for the diagnosis and treatment of AR.Methods Thirty patients with allergic rhinitis were included as the experimental group and 20 healthy children as the control group.The levels of CD4+CD25+CD127-Treg cells in peripheral blood were detected by flow cytometry(FCM),and the serum levels of cytokines associated with Th17,Th1 and Th2 cells were detected by flow cytometry(CBA).Results The contents of CD4+CD25+CD127-Treg cells in AR group was significantly lower than that in normal controls(P<0.001).Serum levels of TH2-related cytokines IL-6 and IL-10 in AR patients were significantly increased(P<0.05),and serum levels of IL-17 were significantly lower than those of normal subjects(P<0.05).In the patient group,IL-2 was positively correlated with TNF-α(r=0.665,P<0.001)and IL-4(r=0.440,P<0.05).IFN-was positively correlated with IL-10(r=0.568,P<0.05).Conclusion There is an obvious imbalance between Th17 cells and Treg cells in serum of AR patients.The imbalances of Th17/Treg and Th1/Th2 cells and the interaction of their secreted cytokines play a role in the pathogenesis of AR.
