摘  要:目的探讨双侧经横突-小关节突改良入路骨水泥锚定技术结合体外复位法治疗新鲜骨质疏松性椎体压缩骨折(OVCF)的临床效果。方法回顾性分析我院2016年1月至2017年7月收治的30例(30椎)新鲜OVCF患者的临床资料,患者均采用双侧经横突-小关节突改良入路骨水泥锚定技术结合体外复位法治疗。比较患者手术前及手术后2 d、2个月的椎体高度、Cobb角、Oswestry功能指数(ODI)评分、视觉模拟评分法(VAS)评分。结果30例患者均顺利完成手术。术后72 h内,30例患者腰背部或腰骶部疼痛均明显缓解,下地行走自如,日常活动能力均明显改善。患者术后随访6~12个月,平均(7.8±2.0)个月。术后2 d及2个月,伤椎前1/3高度、伤椎中1/3高度、伤椎后1/3高度均较术前明显升高,Cobb角、ODI评分、VAS评分均较术前明显降低,差异具有统计学意义(P<0.05)。术后2个月的VAS评分明显低于术后2 d,差异具有统计学意义(P<0.05)。结论双侧经横突-小关节突改良入路骨水泥锚定技术结合体外复位法治疗新鲜OVCF的临床效果显著,可缓解患者疼痛,改善伤椎高度,矫正后凸畸形,具有较高的患者满意度。Objective To explore the clinical effect of bone cement anchoring technology bilateral transverse process-facet joint process modified approach combined with external reduction in the treatment of fresh osteoporotic vertebral compression fracture(OVCF).Methods The clinical data of 30 patients(30 vertebrae)with fresh OVCF admitted in our hospital from January 2016 to July 2017 were retrospectively analyzed,and all the patients were treated with bone cement anchoring technology bilateral transverse process-facet joint process modified approach combined with external reduction.The vertebral height,Cobb angle,Oswestry function index(ODI)score and visual analogue scale(VAS)score were compared before operation and 2 d and 2 months after operation.Results All 30 patients successfully completed the operation.Within 72 h after operation,the pain in the back or lumbosacral region of 30 patients was significantly relieved,the patients can walk freely on the ground,and the ability of daily activities was significantly improved.The patients were followed up for 6 to 12 months,with an average of(7.8±2.0)months.At 2 d and 2 months after operation,the height of anterior 1/3 of the injured vertebra,the height of middle 1/3 of the injured vertebrae and the height of posterior 1/3 of the injured vertebrae were significantly higher than those before the operation,and the Cobb angle,ODI score and VAS score were significantly lower than those before operation(P<0.05).The VAS score at 2 months after operation was significantly lower than that at 2 days after operation,and the difference was statistically significant(P<0.05).Conclution Bone cement anchoring technology bilateral transverse process-facet joint process modified approach combined with external reduction in the treatment of fresh OVCF has a significant clinical effect,which can relieve pain of patients,improve the height of the injured vertebrae,correct kyphotic deformity,and has high patients'satisfaction.
