摘 要:目的探讨早产儿脑干听觉诱发电位(BAEP)的特征及Ⅴ波反应阈值的情况。方法抽取2017年11月至2018年11月郑州大学附属儿童医院收治的不同出生胎龄早产儿56例为观察组,另选取同期正常新生儿36例为对照组。对两组新生儿进行BAEP及Ⅴ波反应阈值检测,分析BAEP主要波形成分Ⅰ、Ⅲ、Ⅴ波的峰值潜伏期、波幅及Ⅰ~Ⅲ、Ⅲ~Ⅴ、Ⅰ~Ⅴ波峰间期,以Ⅴ波反应阈值作为听阈。结果观察组中BAEP异常者54例,异常率为96.4%;对照组中BAEP异常者4例,异常率为11.1%,差异有统计学意义(P<0.05)。异常情况主要表现为Ⅰ、Ⅲ、Ⅴ波部分或全部消失,各波潜伏期延长,Ⅲ~Ⅴ或Ⅰ~Ⅲ波峰间期延长,Ⅴ波反应阈增高等。观察组存在听力损伤34例(60.7%,34/56),对照组存在听力损伤3例(8.3%,3/36),差异有统计学意义(χ2=25.0,P<0.01)。观察组中,出生体质量<1 000 g的8例早产儿均发生听力损伤,出生体质量1 001~1 500 g的28例早产儿中听力损伤18例,出生体质量1 501~2 500 g的20例早产儿中听力损伤8例。结论 BAEP检查有助于早产儿脑损伤的早期诊断及听力评估,体质量是影响早产儿BAEP的重要因素之一,体质量愈小,听阈越易受损。Objective To investigate the characteristics of brainstem auditory evoked potential(BAEP)and the threshold of V wave response in premature infants.Methods A total of 56 premature infants with different gestational ages admitted to Children’s Hospital of Zhengzhou University from November 2017 to November 2018 were selected as the observation group,and 36 normal newborns were selected as the control group.BAEP and V wave response threshold of the two groups were detected.The peak latencies and amplitudes of wavesⅠ,ⅢandⅤof BAEP,and the inter peak latencies of wavesⅠ-Ⅲ,Ⅲ-ⅤandⅠ-Ⅴwere analyzed,V wave response threshold was used as the threshold of hearing.Results In the observation group,54 cases had abnormal BAEP,the abnormal rate was 96.4%;in the control group,4 cases had abnormal BAEP,the abnormal rate was 11.1%,and the difference was statistically significant(P<0.05).The abnormal situation mainly manifested as partially or completely disappeared waveⅠ,Ⅲand V,prolongation of the peak latency of each wave,prolongation of the peak intervals of waveⅢ-Ⅴor waveⅠ-Ⅲ,and theⅤ-wave response threshold increasing.There were 34 cases of hearing loss in the observation group(60.7%,34/56)and 3 cases(8.3%,3/36)in the control group,and the difference was statistically significant(χ2=25.0,P<0.01).In the observation group,8 premature infants with birth weight<1000 g had hearing loss,18 of 28 premature infants with birth weight of 1001-1500 g had hearing loss,and 8 of 20 premature infants with birth weight from 1501 g to 2500 g had hearing loss.Conclusions BAEP examination is helpful for early diagnosis and hearing assessment of premature infants with brain injury.Body mass is one of the important factors affecting BAEP of premature infants.The smaller the body weight is,the more vulnerable the hearing threshold gets.
