鼠李糖乳杆菌对结肠切除术后大鼠肠道功能、愈合情况和免疫状态的影响

摘　　要：目的通过大鼠结肠切除术构建的短肠综合征(short bowel syndrome,SBS)损伤模型研究鼠李糖乳杆菌(Lactobacillus rhamnosus Gorbach-Goldin,LGG)对大鼠肠道功能、愈合情况及免疫状态的影响。方法30只6周龄雄性Sprague-Dawley大鼠随机分为假手术组、模型组和实验组,每组10只。假手术组行结肠切除再吻合术后每日灌胃给予无菌水1 mL,模型组和实验组行左半结肠切除术后每日灌胃给予无菌水1 mL或5×10^8 CFU活性LGG。术后第14天处死大鼠,观察大鼠术后的体重、肠通透性、细菌转移率和炎性细胞因子表达等指标。结果假手术组、模型组和实验组的体重分别为(260.92±7.54),(190.37±7.35),(213.88±7.16)g,肠系膜淋巴结细菌转移率分别为10%,70%,10%,肝脏细菌转移率分别为0,30%,0,脾脏细菌转移率分别为0,40%,0,血清FD-40浓度分别为(0.25±0.06),(1.52±0.29),(0.28±0.07)μg·mL-1,血清内毒素水平分别为(1.52±0.22)×10^-2,(2.85±0.36)×10^-2,(1.73±0.51)×10^-2 EU·mL^-1,分泌型免疫球蛋白A(SIgA)分别为(65.22±6.03),(24.89±4.96),(60.13±5.73)ng·mL^-1,肿瘤坏死因子-α(TNF-α)水平分别为(60.53±10.84),(450.92±34.47),(132.78±28.65)pg·mL^-1,白细胞介素-6(IL-6)水平分别为(150.41±23.62),(510.47±32.18),(243.33±29.39)pg·mL^-1。上述指标,模型组分别与假手术组和实验组比较,差异均有统计学意义(均P<0.05)。结论肠内补充LGG可改善结肠切除术后SBS的肠屏障功能障碍,促进术后愈合并维持肠道功能和免疫状态的稳定性。Objective To explore the effects of Lactobacillus rhamnosus Gorbach-Goldin(LGG)on intestinal function,healing and immune status in rats with short bowel syndrome(SBS)injury model established by colectomy.Methods Thirty six-week-old male Sprague-Dawley rats were randomly divided into sham group,model group and test group(n=10 per group).Sham group submitted to colectomy and reanastomosis and then supplemented with 1 mL of sterile water by daily gavage.Model group and test group submitted to left hemicolectomy then supplemented with either 1mL sterile water or 5×10^8 CFU viable LGG by daily gavage.Rats were sacrificed on day 14 postoperation.Body weight,intestinal permeability,bacterial translocation,and inflammatory cytokines expressions of ileum were evaluated.Results The body weight in sham group,model group and test group were(260.92±7.54),(190.37±7.35),(213.88±7.16)g,the bacterial translocation rate of mesenteric lymph nodes in the three groups were 10%,70%,10%,the bacterial translocation rate of liver in the three groups were 0,30%,0,the bacterial translocation rate of spleen in the three groups were 0,40%,0,the serum FD-40 concentration in the three groups were(0.25±0.06),(1.52±0.29),(0.28±0.07)μg·mL^-1,the serum endotoxin in the three groups were(1.52±0.22)×10^-2,(2.85±0.36)×10^-2,(1.73±0.51)×10-2EU·mL^-1,the secretory immunoglobulin A(SIgA)in the three groups were(65.22±6.03),(24.89±4.96)and(60.13±5.73)ng·mL^-1,the levels of tumor necrosis factorα(TNF-α)in the three groups were(60.53±10.84),(450.92±34.47)and(132.78±28.65)pg·mL^-1,the levels of interleukin-6(IL-6)in the three groups were(150.41±23.62),(510.47±32.18)and(243.33±29.39)pg·mL^-1.There were statistically significant differences between the model group and the sham/test group in the above indexes(P<0.05).Conclusion Enteral supplementation of LGG can improve the intestinal barrier dysfunction of SBS after colectomy and promote postoperative recovery and maintain the stability of intestinal function and immune state.
