摘 要:目的探讨高分辨率CT与三维重建技术诊断鼻创伤的应用价值。方法选取我院2017年1月—2019年12月诊治的鼻创伤患者100例,均行高分辨率CT检查,高分辨率CT难以确诊病例进行三维重建,分析诊断结果。结果高分辨率CT诊断检出鼻骨及相邻结构骨折84例,检出率为84.00%;其中,鼻骨骨折检出70例,单纯性鼻中隔骨折检出6例,鼻颌缝分离检出4例,鼻中隔软骨脱位检出4例。70例鼻骨骨折中,单侧鼻骨骨折44例,双侧鼻骨骨折26例;粉碎性鼻骨骨折9例,合并鼻中隔骨折38例,合并上颌窦前壁骨折13例,合并上颌骨额突骨折10例。对16例高分辨率CT诊断难以确诊病例进行三维重建,确诊骨折及相邻结构骨折14例,排除骨折2例。其中,单纯性鼻骨骨折2例,合并鼻中隔骨折6例,合并上颌窦前壁骨折4例,合并上颌骨额突骨折2例。结论高分辨率CT与三维重建技术诊断鼻创伤在检出率上均较高,能够清楚显示鼻部骨质结构,判断单纯骨折与复合骨折,为临床诊断、治疗提供准确参考依据,具有较高的应用价值。Objective To explore the value of high-resolution CT and three-dimensional reconstruction in the diagnosis of nasal trauma.Methods from January 2017 to December 2019,100 patients with nasal trauma were selected,all of them were examined with high-resolution CT,and the three-dimensional reconstruction was performed for the difficult cases of high-resolution CT,and the diagnosis results were analyzed.Results 84 cases of fracture of nasal bone and adjacent structures were detected by high resolution CT,the detection rate was 84.00%.Among them,70 cases of nasal bone fracture,6 cases of simple nasal septum fracture,4 cases of nasomaxillary suture separation and 4 cases of cartilage dislocation of nasal septum were detected.Among 70 cases of nasal bone fracture,44 cases were unilateral,26 cases were bilateral;9 cases were comminuted,38 cases were combined with septal fracture,13 cases were combined with anterior wall fracture of maxillary sinus,and 10 cases were combined with frontal process fracture of maxilla.Three dimensional reconstruction was performed in 16 cases of high-resolution CT,14 cases of fracture and adjacent structure fracture were confirmed,and 2 cases of fracture were excluded.Among them,there were 2 cases of simple nasal bone fracture,6 cases of nasal septum fracture,4 cases of maxillary sinus anterior wall fracture and 2 cases of maxillary frontal process fracture.Conclusion High-resolution CT and three-dimensional reconstruction have high detection rate in the diagnosis of nasal trauma,which can clearly show the nasal bone structure,judge the simple fracture and complex fracture,provide accurate reference for clinical diagnosis and treatment of nasal trauma,and have high application value.
