任绪东“补肾活血开窍法”治疗中风痴呆经验  

REN Xudong’s Experienceon Treating Dementia of Stroke with Method of Tonifying Kidney and Invigorating Blood Circulation and Opening Orifices

摘　　要：任绪东主任医师认为中风痴呆因情志不遂、五志内伤或年老体衰,脏腑功能失调,致脑髓空虚,痰瘀内生,上闭清窍所致,该病与痰、瘀、虚有关,且三者互相影响,共同致病。提出"凡呆者必因正虚邪气(痰瘀)闭窍所致",认为该病根本病机为肾虚痰瘀闭窍,主张以补肾活血化痰开窍为基本治法,以此理论创立经验方"开窍益智方"治疗本病,所附临床医案1例,以"开窍益智方"加减治疗中风痴呆辨证为肾虚痰瘀阻窍者。在临证时体现病初以通滞为主,兼顾补虚;病久者以补虚为主,兼通其滞,遣方用药重视醒脑开窍药物及破血逐瘀药物的应用。Physician REN Xudong thinks that dementia of stroke is due to empty of brain marrow,endogenous phlegm and blood stasis andobstruction of the clear orifices,which were caused by emotional paralysis,mental trauma,physical recession of aging and dysfunction of viscera.He also considers that the disease is often accompanied with phlegm,blood stasis and deficiency of Qi.The three factors affect each other and cause this disease together.REN Xudong presents an idea that all dementia patients are caused bydeficiency of Qi and obstruction of the clear orifices(phlegm and blood stasis).The basic etiology and pathogenesis of dementia is kidney deficiency,phlegm and blood stasis.He suggests that tonifying kidney,activatingblood circulation,resolving phlegm and opening the orifice should be the basic treatment.Based on this theory,experience prescription"Kaiqiao Yizhi Recipe"has been established to treat this disease.There was a clinical case which used Kaiqiao Yizhi Recipe in the treatment ofdementia of kidney deficiency and phlegm stasis syndrome.In clinical manifestation of this disease,dredging method is the main treatment at the beginning,giving consideration to tonifying.Tonifying deficiency should be the main treatment for long-term illness.When giving a prescription,the attention should be paid to the application of drugs for restoring consciousnessand removing blood stasis.
