益气通络活血汤联合西药治疗气虚血瘀型糖尿病周围神经病变疗效分析

摘　　要：目的研究与分析益气通络活血汤联合西药治疗气虚血瘀型糖尿病周围神经病变疗效。方法将2017年1月-2019年12月收治的96例气虚血瘀型糖尿病周围神经病变患者,采用随机数字表法分为观察组和对照组各48例。对照组在控制血压、血糖的基础上予以甲钴胺,0.5 mg/次,3次/d,治疗2个月。观察组在对照组基础上加用益气通络活血汤,水煎400 mL,1剂/d,早晚温服,治疗2个月。观察两组治疗后临床疗效,治疗前后中医证候积分、多伦多临床(TCSS)评分、运动神经传导速度(MCV)、感觉神经传导速度(SCV)。结果治疗后,观察组总有效率高于对照组(P<0.05);治疗后,两组中医证候积分均有降低与治疗前比较差异显著(P<0.05),与此同时观察组中医证候积分明显低于对照组(P<0.05);治疗后,两组TCSS评分均有降低,与治疗前比较差异显著(P<0.05),两组组间比较观察组更低且差异显著(P<0.05);治疗后,两组运动神经传导速度、感觉神经传导速度均显著升高(P<0.05)且观察组变化更明显,腓总神经分别达到(46.29±7.85)m/s、(45.78±5.37)m/s,正中神经分别达到(45.07±4.29)m/s、(35.19±3.14)m/s与对照组(43.26±7.26)m/s、(42.56±5.19)m/s与(42.26±4.07)m/s、(33.26±3.20)m/s比较差异显著(P<0.05)。结论益气通络活血汤联合西药治疗气虚血瘀型糖尿病周围神经病变可提高临床疗效,改善症状及体征,有助于受损神经功能的修复,值得推广。Objective To study and analyze the therapeutic effect of Yiqi Tongluo Huoxue decoction combined with western medicine on diabetic peripheral neuropathy of Qi deficiency and blood stasis type.Methods 96 patients with diabetic peripheral neuropathy of Qi deficiency and blood stasis type admitted in the hospital from January 2017 to December 2019 were randomly divided into observation group and control group with 48 cases in each group.The control group was treated with mecobalamin on the basis of controlling blood pressure and blood glucose,0.5 mg/time,3 times/day,for 2 months.On the basis of the control group,the observation group was treated with Yiqi Tongluo Huoxue decoction,400 m L,1 dose/d,taking in the morning and evening for 2 months.The clinical effect,TCM syndrome score,Toronto clinical(TCSs)score,motor nerve conduction velocity(MCV)and sensory nerve conduction velocity(SCV)were observed before and after treatment.Result After treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the motor nerve conduction velocity and sensory nerve conduction velocity of the two groups increased significantly(P<0.05),and the changes of the observation group were more obvious.The total peroneal nerve reached(46.29±7.85)m/s and(45.78±5.37)m/s,respectively.The median nerve reached(45.07±4.29)m/s and(35.19±3.14)m/s respectively.The control group’s total peroneal nervereached(43.26±7.26)m/s and(42.19±3.14)m/s and(42.56±5.19)m/s and the median nerve reached(42.26±4.07)m/s and(33.26±3.20)m/s(P<0.05).Conclusion Yiqi Tongluo Huoxue decoction combined with western medicine can improve the clinical effect,the symptoms and signs,and help to repair the damaged nerve function,and it is worthy of popularizing.
