摘 要:目的分析重症监护病房(ICU)与普通病房(非ICU)病原菌耐药性及多重耐药菌(MDRO)检出率,为临床抗感染治疗及MDRO防控提供参考。方法对本院2016年1月-2017年12月住院患者中分离到的4种目标监测菌,按ICU和非ICU进行耐药性和MDRO检出率分析。结果ICU大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌、鲍曼不动杆菌MDRO检出率分别为62.5%、38.2%、64.0%、82.1%,非ICU分别为43.7%、25.5%、17.4%、31.5%;大肠埃希菌的耐药率(氨苄西林、庆大霉素、阿米卡星除外)ICU高于非ICU(P<0.05);铜绿假单胞菌的耐药率(庆大霉素、阿米卡星除外)ICU高于非ICU(P<0.05);肺炎克雷伯菌、鲍曼不动杆菌的耐药率ICU明显高于非ICU(P<0.05)。结论4种目标监测菌ICU比非ICU有更高的耐药率和MDRO检出率,应加强ICU感染的防控。Objective To analyze the drug resistance and detection rates of multidrug-resistant organism(MDRO)in intensive care unit(ICU)and general ward(non-ICU),so as to provide reference for clinical anti-infection treatment and prevention and control of MDRO.Methods Four target monitoring bacteria were isolated from inpatients between January 2016 and December 2017,drug resistance and detection rates of MDRO were analyzed and compared between ICU and non-ICU.Results The detection rates of MDRO in Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa and Acinetobacter baumannii were 62.5%,38.2%,64.0%,and 82.1%,respectively for ICU,and 43.7%,25.5%,17.4%,31.5%for non-ICU.The resistance rates of Escherichia coli(except ampicillin,gentamicin and amikacin)in ICU were higher than that of non-ICU(P<0.05);the resistance rates of Pseudomonas aeruginosa(except gentamicin and amikacin)were higher than that of non-ICU(P<0.05);the resistance rates of Klebsiella pneumoniae and Acinetobacter baumannii in ICU were higher than non-ICU(P<0.05).Conclusion The drug resistance and detection rates of MDRO in ICU were higher than that in non-ICU for four target monitoring bacteria,and the prevention and control of ICU infection should be strengthened.
