摘　　要：目的探讨中性粒细胞与淋巴细胞比值和血清淀粉样蛋白A(SAA)与2型糖尿病(T2DM)并发冠心病的相关性。方法选取T2DM患者550例,根据患者动脉造影的结果将其分为单纯的T2DM组360例和T2DM-CHD组190例,另选取健康体检者300例作为对照组。比较3组患者静脉血中中性粒细胞和淋巴细胞比值及SAA表达水平,采用Logistic回归分析探讨T2DM合并冠心病的相关因素。结果T2DM-CHD、T2DM组NLR和SAA的水平均高于对照组(P<0.05),T2DM-CHD组NLR和SAA的水平高于T2DM组(P<0.05);Logistic回归分析结果显示,TG、NLR和SAA是糖尿病并发冠心病的危险因素(OR值分别为1.219、3.279和2.798,P<0.05)。结论NLR和SAA表达水平的升高与T2DM患者并发冠心病之间存在明显的关联性,因此可作为T2DM是否并发冠心病及其预后观察的一项重要指标。Objective To investigate the relationship between neutrophil/lymphocyte ratio and SAA and type 2 diabetes complicated with coronary heart disease.Methods A total of 550 patients with T2DM were selected and divided into simple T2DM group(360 cases)and T2DM-CHD group(190 cases)according to the results of arteriography.Another 300 healthy people were selected as the control group.The ratio of neutrophils to lymphocytes and the expression of SAA in venous blood of the three groups were compared.Logistic regression analysis was used to explore the related factors of T2DM complicated with coronary heart disease.Results The levels of NLR and SAA in the three groups were higher than those in the control group(P<0.05).The levels of NLR and SAA in the T2DM-CHD group were higher than those in the T2DM group(P<0.05).Logistic regression analysis showed that TG,NLR and SAA were risk factors for diabetes mellitus complicated with coronary heart disease(OR=1.219,OR=3.279,OR=2.798,P<0.05).Conclusion Increased expression of NLR and SAA is associated with coronary heart disease in patients with T2DM.Therefore,it can be used as an important index to observe whether T2DM is complicated with coronary heart disease and its prognosis.
