中医综合治疗糖尿病周围神经病变临床研究

摘　　要：目的观察芪黄汤内服联合糖痹外洗方沐足治疗2型糖尿病周围神经病变(Diabetic peripheral neuropathy,DPN)的有效性及安全性。方法观察2016年6月—2018年7月在广东省中医院糖尿病专科病房的70例辨证属于气阴两虚脉络瘀阻的DPN患者,采用前瞻性、随机对照的研究方案,按随机数字表法分为两组:35例治疗组接受西医基础治疗+芪黄汤内服+糖痹外洗方沐足,35例对照组接受西医基础治疗,西医基础治疗包括:常规降糖以及弥可保静推营养神经、硫辛酸静滴抗氧化治疗。观察2周。比较用药前后两组症状、多伦多评分、生活质量量表评分的变化情况。数据采用t检验或χ^2检验进行比较分析。结果治疗后治疗组能提高正中神经、腓肠神经的感觉神经传导速度,差异有统计学意义(P<0.05);治疗后治疗组较对照组麻木、疼痛、感觉异常均有改善,差异有统计学意义(P<0.05);两组治疗后多伦多评分较治疗前下降(P<0.05),且治疗组下降较对照组更明显,差异有统计学意义(P<0.05);两组治疗后生活质量量表评分较治疗前升高(P<0.05),且治疗组升高较对照组更明显,差异有统计学意义(P<0.05)。结论中医内外综合治疗能安全、有效改善DPN患者麻木、疼痛、感觉异常的临床症状,改善多伦多评分,提高生活质量,提高感觉神经传导速度,值得临床推广应用。Objective To investigate the effect and safety of taking Qihuang Decoction orally and Tongbi External Washing Formula for foot washing on diabetic peripheral neuropathy in patients with type 2 diabetes mellitus.Methods Seventy patients with syndrome differentiation of deficiency of both Qi and Yin and stasis in collaterals in wards of Endocrinology Department in Guangdong Province Hospital of Traditional Chinese Medicine from June 2016 to July 2018 were enrolled in this study.Those patients were randomly divided into two groups according to random number table.Thirty-five patients in the test group were treated with basic treatment of western medicine,Qihuang Decoction orally and Tangbi External Washing Formula for foot washing while 35 patients in the control group were treated with basic treatment of western medicine including hypoglycemic medication and Mecobalamin for nourishing nerve and Lipoic acid for antioxidation.All patients were under observation of two weeks.The changes of symptoms,Toronto scores and quality of life scores were compared between the two groups before and after treatment.The t test andχ^2 test were used in data analysis.Results After treatment,the treatment group can improve the sensory nerve conduction velocity of the median nerve and sural nerve and the difference was statistically significant(P<0.05).After treatment,the numbness,pain and abnormal sensation of the treatment group were alleviated compared with those of the control group(P<0.05).The Toronto score of the two groups decreased after treatment(P<0.05),and the decrease of the treatment group was more obvious than that of the control group(P<0.05).The scores of quality of life of the two groups were higher after treatment(P<0.05)and the scores in the treatment group were higher than those in the control group(P<0.05).Conclusion TCM internal and external treatment can safely and effectively alleviate the clinical symptoms of numbness,pain and sensory abnormalities in DPN patients,and improve the Toronto scores,the quality of
