摘 要:目的探讨胎儿超声用于产前筛查先天性心脏畸形的价值。方法抽取郑州人民医院2018年1月至2019年1月行胎儿超声产前筛查的300例孕妇为研究对象,回顾性分析胎儿先天性心脏畸形检出情况,并随访产后新生儿先天性心脏畸形的发生情况。结果在300例接受胎儿超声产前筛查的孕妇中,筛查出先天性心脏畸形者15例,检出率为5.00%,其中单纯性室间隔缺损5例,法洛四联症3例,心内膜垫缺损2例,大动脉转位1例,左心发育不良1例,右心发育不良1例,三尖瓣下移1例,右室双出口1例。产后随访,先天性心脏畸形16例,胎儿超声产前筛查诊断符合率为93.75%,单纯性室间隔缺损漏诊1例,诊断灵敏度为93.75%,特异度为100.00%。结论胎儿超声产前筛查在先天性心脏畸形诊断中有较高的准确性,其可及时发现胎儿先天性心脏畸形,有效提高出生人口质量。Objective To explore the application value of fetal ultrasound prenatal screening in congenital heart malformation screening.Methods A total of 300 pregnant women who underwent fetal ultrasound prenatal screening in Zhengzhou People’s Hospital from January 2018 to January 2019 were selected as the research subjects,and the detection of fetal congenital heart malformation was analyzed retrospectively,and the incidence of postpartum newborns congenital heart malformation was followed up.Results Among the 300 pregnant women who received fetal ultrasound prenatal screening,15 cases of congenital heart malformation were detected,and the detection rate of congenital heart malformation was 5.00%;there were 5 cases of simple ventricular septal defect,3 cases of tetralogy of Fallot,2 cases of endocardial cushion defect,1 case of transposition of great artery,1 case of left ventricular dysplasia,1 case of right ventricular hypoplasia,1 case of tricuspid valve depression and 1 case of double outlet of right ventricle.During the postpartum follow-up,there were 16 cases of congenital heart malformation.The coincidence rate of prenatal ultrasound screening was 93.75%.One case of simple ventricular septal defect was missed.The diagnostic sensitivity was 93.75%,and the specificity was 100.00%.Conclusions Fetal ultrasound prenatal screening has a high accuracy in the diagnosis of congenital heart malformations,which can detect fetal congenital heart malformations in time and effectively improve the quality of the birth population.
