摘 要:目的探究卡式微柱凝胶法联合聚凝胺法配血预防输血反应的效果。方法抽取2019年3月至2019年12月侯马市人民医院中心血库输血320例患者作为研究对象,根据配血方法不同将其分为A组(107例)、B组(107例)、C组(106例),其中A组采用卡式微柱法配血,B组采用聚凝胺法配血,C组采用两种联合配血,对配血试验期间的阳性患者采取不完全抗体检测,比较不同交叉配血方法的差异。结果三组主侧阳性、总阳性率与假阳性率比较差异未见统计学意义(P>0.05);三组次测阳性率比较,差异有统计学意义(P<0.05),且C组高于A、B组,差异有统计学意义(P<0.05)。C组灵敏度(36.42%)、特异度(84.23%)、符合率(79.24%)优于A组和B组的灵敏度(3.43%、3.54%)、特异度(77.83%、80.02%)、符合率(15.91%、39.22%)。经不规则抗体检测,A组7例患者中检出不规则抗体4例(均为次测凝聚患者),主侧3例阳性患者诊断为纤维蛋白过多(假阳性);B组17例阳性患者中检出不规则抗体15例,主侧2例患者诊断为红细胞悬液浓度过高(假阳性)。结论凝聚胺法假阳性率较高,卡式微柱凝胶法操作难度低,两种联合检测在不规则抗体筛查中敏感性高。Objective To explore the clinical effect of gel system combined with polybrene method for blood transfusion to prevent transfusion reaction.Methods From March 2019 to December 2019,320 patients from Central Blood Bank of Houma People’s Hospital were enrolled.According to the different blood distribution methods,they were divided into group A(107 cases),group B(107 cases),and group C(106 cases).Group A was matched with gel system,group B was matched with polybrene method,and group C was matched with two methods.The incomplete antibody detection was used in positive patients during the blood test.The differences between the different cross blood matching methods were compared.Results There were no statistically significant differences in the primary positive,total positive and false positive rates among the three groups(P>0.05).There was significant difference in the secondary positive among the three groups(P<0.05),and that in group C was higher than that in group A and group B(P<0.05).The sensitivity(36.42%),specificity(84.23%),agreement(79.24%)in group C were better than those in group A and group B[(3.43%,3.54%),(77.83%,80.02%),(15.91%,39.22%)].According to the detection of irregular antibodies,4 out of 7 patients in group A were detected with irregular antibodies(all with secondary agglutination),3 primary positive patients were diagnosed with excessive fibrin(false positive);15 of 17 positive patients in group B were with irregular antibodies,and 2 primary positive patients were with high erythrocyte suspension concentration(false positive).Conclusions The false positive rate of the polybrene method is high,while the operation of the gel system is easy,and the two joint tests have high sensitivity in the screening of irregular antibodies.
