MR常规扫描及功能成像在颅脑单发转移瘤诊断中的应用研究

摘　　要：目的探讨MR常规扫描及功能成像(MRS、DWI及SWI)在颅脑单发转移瘤诊断与鉴别诊断中的应用价值。方法收集经手术或活检病理证实的单发颅脑转移瘤20例。所有病例均行术前常规MR平扫与增强检查,其中20例单发转移瘤中有15例行MRS检查、17例行ADC值测定、13例行SWI检查。并对本组结果进行统计学分析。结果MR平扫7例可见少量出血;MR增强扫描有14例呈较为规则完整的环形强化,16例瘤周见大面积水肿带;MRS提示单发脑转移瘤实质区Cho峰升高,Cr、NAA峰不同程度降低,9例病灶未见明显NAA峰,瘤周水肿区谱线大致正常;单发脑转移瘤实质区的ADC值明显降低,瘤周水肿区ADC值轻度降低或大致正常。SWI显示肿瘤内可见少许点状微小出血灶及静脉血管影。结论MR常规扫描及功能成像(MRS、ADC及SWI)在颅脑单发脑转移瘤的诊断与鉴别诊断中有重要的临床价值。Objective To investigate the value of MR routine examinations and functional imagings(MRS,DWI and SWI)in the diagnosis and differential diagnosis of single brain metastatic tumor.Methods 20 cases of craniocerebral metastatic tumor confirmed by operation or biopsy were collected in the study.Unenhanced and enhanced preoperative MR sc annings were routinely performed for all the patients,including 15 MRS,17 ADC and 13 SWI examinations.The results were statistically analyzed.Results Unenhanced MR examinations showed a litle bleeding in 7 cases.Enhancement MR examinations showed relatively regular and complete ring-shaped enhancement in 14 cases,and large area of edema in 16 cases.MRS suggested that Cho peak in parenchymal area of single brain metatatic tumor was increased,Cr and NAA peaks were decreased to different degrees,no significant NAA peak was observed in 9 cases of lesions,and the spectral line of peritumor edema was generally normal.ADC values in the parenchyma of single brain metastatic tumor significantly decreased,and ADC values in the peritumor edema area slightly decreased or were approximately normal.SWI showed a few punctured mic rohemorrhages and venous shadows in the tumors.Conclusion MR routine examinations and functional imagings(MRS,ADC and SWI)have important clinical value in the diagnosis and differential diagnosis of single brain metastatic tumor.
