类风湿性关节炎住院患者疼痛评分骨密度与临床指标相关性研究

摘　　要：目的探讨类风湿性关节炎(RA)疼痛及骨密度(BMD)与相关生化指标的相关性。方法采用量表对患者治疗前后疼痛进行评分,采用双能X线骨密度仪,测量RA住院患者的腰椎、股骨颈、Ward's三角区等的BMD,并同时检测相关临床指标。结果疼痛与血液中皮质醇、血沉(ESR)、C反应蛋白(CRP)、中性粒细胞比例呈正相关(P<0.05),与抗环瓜氨酸肽(CCP)抗体及白细胞计数有弱相关性(P>0.05);RA患者腰椎L2-L4、股骨大粗隆、股骨干与病程呈负相关(P<0.05),L2-L4、Ward's三角区、股骨大粗隆与类风湿因子(RF)呈负相关(P<0.05)。结论RA患者经过住院治疗后,疼痛评分降低,ESR、CRP、皮质醇、中性粒细胞比例降低。血液中皮质醇、ESR、CRP、中性粒细胞比例与疼痛呈正相关。RA患者大部分腰椎、股骨大粗隆和股骨干BMD值降低,与病程、RF有负相关性。Objective To explore the correlation between pain,BMD in rheumatoid arthritis(RA)and related biochemical indexes.Methods Graded pain before and after treatment with a scale.BMD of lumbar spine,femoral neck and Ward's trigonometry of RA inpatients were measured with a dual-energy X-ray absorptiometry,and relevant clinical indicators were detected at the same time.Results Pain was positively correlated with the proportion of cortisol,ESR,CRP and neutrophils in the blood(P<0.05),and weakly correlated with CCP and white blood cell count(P>0.05).In RA patients,lumbar L2-L4,femoral greater trochanter and femoral shaft were negatively correlated with the course of disease(P<0.05),while L2-L4,Ward's triangle and femoral greater trochanter were negatively correlated with RF(P<0.05).Conclusion After hospitalization,the pain score of RA patients decreased,and the proportion of ESR,CRP,cortisol and neutrophils decreased.The proportion of cortisol,ESR,CRP and neutrophils in the blood was positively correlated with the pain.BMD value of lumbar spine,femoral greater trochanter and femoral shaft decreased in most RA patients,which was negatively correlated with course of disease and RF.
