肥胖对OSAHS患者甲状腺自身免疫性损伤的作用

摘  要:目的探讨肥胖与阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者甲状腺自身免疫损伤关系及对其影响作用。方法筛查出OSAHS伴肥胖患者40例,OSAHS不伴肥胖患者40例,肥胖无OSAHS患者40例,测定各组患者体质量指数(BMI)、呼吸暂停低通气指数(AHI)、夜间最低血氧饱和度、抗体TPO-1、抗体TG-1、甲状腺素、血清瘦素水平,各组间相互比较及有差异指标相关性分析。结果OSAHS伴肥胖组、OSAHS不伴肥胖组患者较单纯肥胖组患者TPO-1更高(P<0.01);OSAHS伴肥胖组瘦素水平较OSAHS不伴肥胖组及单纯肥胖组患者更高(P<0.01);各组患者TG-1、甲状腺素均无明显差异;OSAHS伴肥胖组患者TPO-1与AHI、血清瘦素水平均呈明显相关性(P<0.05,<0.01)。结论OSAHS伴肥胖患者甲状腺自身免疫性损伤与AHI、肥胖有关,患者甲状腺自身免疫损伤发病机制可能与瘦素水平变化有关。Objective To investigate the relationship between obesity and thyroid autoimmune injury in patients with OSAHS and its effects.Methods 40 patients with OSAHS and obesity were enrolled,40 patents with OSAHS without obesity,and 40 patients with obesity without OSAHS.There were 36 males and 4 females in each group.Body mass index(BMI)and respiration were measured in each group.Suspension of hypopnea index(AHI),nocturnal minimum oxygen saturation,serum TPO-1,TG-I,thyroxine,serum leptin levels,comparison between groups and correlation analysis of differences.Results The OSOHS with obesity group and OSAHS without obesity group had higher TPO-1 than the obese group(P<0.01).OSAHS with obesity group had lower leptin level than OSAHS without obesity group and simple obese group.There were significant differences(P<0.01).There was no significant difference in TG-I and thyroxine between the groups.There was a significant correlation between TPO-1 and AHI and serum leptin levels in OSAHS patients with obesity(P<0.05),<0.01).Conclusion The autoimmune injury of thyroid in OSAHS with obesity is related to AHI and obesity.The pathogenesis of thyroid autoimmune injury may be related to the change of leptin level.
